0054612758 2017-07-19 Production Wells No Violations The well is currently equipped properly to be considered capable of production. The well appears to be free flowing currently with no artificial lift technology in place. The well is piped over to a gas processing unit and then routed to sale and to the tank battery. The tank battery contains six 400 barrel production fluid storage tanks. Company representatives were on site working with the gas processing unit during my inspection. The production records in RBDMS are current to 2017.

0663534002 2018-05-10 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that was open flowing with valves in the open position. Annular pressure monitoring was in place for the 9.625 inch diameter casing gauge read 1000 pounds per square inch, the 13.375 inch diameter casing gauge read 450 pounds per square inch. Production equipment included a separator, Gas Processing unit, line heater all shared with 3401320783, and 4- 400 barrel purge tanks shared with 3401320783. Identification tags were present and production was up to date.
0817733645 2015-01-16 Preliminary Restoration No Violations The preliminary restoration for this well is completed. The pad is in satisfactory condition with proper erosion and sedimentation controls in place. Company shall continue to monitor the location to ensure the pad remains in satisfactory condition. Preliminary restoration passed.
1344159062 2019-01-10 Production Wells No Violations Status check inspection found 2 wells present on the pad and in production. Both wells are equipped with a lower A and upper B section with riser pipes attached to the 2 outlet ports of both sections, the riser pipes are equipped with valves and gauges for the purpose of annular pressure monitoring. The A section pressure was 300 pounds per square inch and the B section pressure was 1,000 pounds per square inch. The 5.5 diameter casing pressure was 300 pounds per square inch and the sales flow line pressure was 200 pounds per square inch. There is a production tree on the B section with no artificial lift present. There is a hydraulic emergency shut down device present on the production tree along with 3 manual gate valves. The production equipment is reduced down to one gas processing unit. There are (6) steel construction 400 barrel capacity production fluid tanks present along with one 50 barrel tank, all situated inside a polymer lined steel containment dike area. The well pad is protected by a earthen berm that is in good repair. The lease road and well pad surface is stone construction and in good repair. There is emergency shut down control points at the well pad entrance along with at the well head and production tank and gas processing areas.
1406078703 2014-07-22 Casing String Witnessed No Violations Onsite to witness cementing of the intermediate string. Sidewinder 121 drilled a 17.5 open hole 2078. Wyoming ran the 2082 of 13 3/8 54.5# casing. Stringray is onsite to cement the casing with 1603 sacks of Class A cement (15.6 ppg; 1.20 yield) using 30% excess and a 44 shoe. Returns were viewed in the half round. Approximately 60 bbls of cement to surface.
