All production mechanics within the tank battery were held within a metal diked area.

1409185271 2014-08-27 Casing String Witnessed No Violations First Class Energy 6 rig drilled a 28x420 open hole and set 407 of 24 (94.5#/ft) casing. Baker hughes in onsite with 605 sacks of class A cement (15.6 ppg; 1.21 yield). Approximately 200 bbl of fresh water was used to displace the cement leaving 20 inside of the casing. Cement returned to surface. Job was delayed due to lightning storm.

The pad is in satisfactory condition with proper erosion and sedimentation controls in place. Company shall continue to monitor the location to ensure the pad remains in satisfactory condition. Preliminary restoration passed.
2041603599 2014-07-26 Casing String Witnessed No Violations Onsite to witness cementing of the conductor. Express Energy services augered a 36 open hole to a depth of 98 and set 95 of 30 conductor pipe. Express Energy Services also cementing the casing back to surface using 275 sacks of class A (15.6 ppg; 1.18 yield) cement at 50% excess. Cement was displaced to 85 using 70 bbl of water but no cement returned to surface. Return tomorrow with grout string.
2103977938 2018-08-07 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design, 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing with no artificial lift technology. Annular pressure monitoring was in place on both the 13.325-inch diameter surface casing and the 9.625-inch diameter deep intermediate casing, pressures were monitored with transducers with no ability to read pressures at the well head. Both casings had bleed down lines ran to the production fluid tanks, at the time of my inspection the 13.375-inch diameter casing was open flowing into tanks through a choke manifold. There were also two chemical vault tanks near the well heads with foaming and anti-scaling agents. Production equipment included a Gas Processing Unit, that was inside of containment. There were also 3- 400-barrel production fluid tanks inside containment. Production fluid tanks were all shared with 34013208120000, 34013208140000. There were also 3 400-barrel production fluid tanks that were on location and not hooked up to the wells. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
