1128535694 2017-10-26 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains five 400-barrel production fluid holding tanks. Production records for this well are current to 2017 within RBDMS.
1413486439 2014-10-16 Casing String Witnessed No Violations Onsite to witness the top out job of the mine string. Inspector Andrew Corder witnessed the bottom job earlier today. Alpha hunter 9 drilled a 28 open hole to a depth of 211 and set 209 of 24 (118 #/ft) casing. Operator stated that no void was encountered but a coal seam was encountered between 167-171. Cement baskets were placed at 151 and 157. Morning pump job did not work as a conventional job as hoped since no void was believed to be encountered. 2.5 bbl of cement was pumped in annulus to bridge of at basket, let set an hour and then 20 bbl of cement was pumped. Visibile water in casing did not appear to rise. Stopped pumping cement. Let set up for 2 hours and tried running 1 down backside to tag up on cement. Have issues getting past 100. Eventually after pulling out of the hole twice got tubing to 145 and pumped 5 bbl to hopefully bridge off at first basket. Set on that for 2 hours and went back to pumping top off. No sign of bridging at basket during next round of pumping. Again tried to set bridge at basket, and waited 2 hours prior to pumping. Called inspector Corder to verify cement to surface in 2 hours.
1413805629 2014-10-16 Casing String Witnessed No Violations On the day of my inspection Alpha Rig 9 had drilled the open hole and the mine string was cemented. No violations noted.

2024606675 2018-09-25 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing with a plunger lift system. Annular pressure monitoring was in place the 9.625-inch diameter intermediate casing gauge read 260 pounds per square inch, the 13.375-inch diameter surface casing gauge read 260 pounds per square inch. Production equipment included a sand trap, Line Heater, Gas Processing Unit. There were also 5- 500-barrel steel production fluid tanks inside secondary containment, that were shared with the rest of the producing wells on the pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was current in RBDMS.
2108629178 2019-02-07 Production Wells No Violations Preliminary Restoration found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad surface and lease road is stone construction and in good repair. The lease road is gated and the well pad has a perimeter earthen berm with a 8 chain link fence on the berm. Signage is posted at each of the 8 wells on the pad. All 8 wells have artificial lift production equipment installed on the production tree. Annular pressure monitoring is in place on the well head A&B sections. The production facility consists of 5 steel tanks capable of holding 500 barrels fluid volume each, the tanks are situated inside a polymer lined steel containment dike area. 8 gas processing units are present on the well pad in 2 different locations.
