0198244251 2018-08-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 4-string casing design and was in normal production. Annular pressure monitoring was in place on both the 9.625-inch diameter casing gauge read 425 pounds per square inch, the 13.375-inch diameter casing gauge read 450 pounds per square inch. Production equipment included an individual Gas Processing unit, and 5- 400-barrel production fluid tanks shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current.

The well is piped to a gas processing unit. From there, it is routed to a production area that contains five 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 429 pounds in between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 600 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. The production records within RBDMS are currently updated to 2017.

1415979456 2014-10-29 Casing String Witnessed No Violations On the date of inspection I was informed by Rice Energy consultant Joe Murray that the Well Service Group 1 rig had drilled a 28 inch diameter hole to a depth of 350 feet. The rig set 24 inch, 79.00lbs/ft casing to a depth of 310 feet. Baker Hughes established circulation and pumped 540sk of Class A cement, 15.6ppg, 1.2 yield, 3% CaCl. Baker Hughes displaced cement with 153bbl of fresh water, no plug or floats. 23bbl of cement returned to surface and stayed at surface while I was on site.
1544030816 2018-12-05 Preliminary Restoration No Violations Preliminary Restoration status check inspection found well pad surface and lease road of stone construction and in good repair. The well pad site does have an earthen berm that appears to be sufficient and preforming with vegetation established. No movement of the outward slopes was observed. There lease road is equipped with a gate and signage is present at the entry point of the lease road along with being posted on each well head. There is a 3rd party midstream point on the left rear corner of the well pad that consists of a contact tower and glycol heater. There are 5 wells on the pad with 5 processing units. All 5 wells were equipped with riser pipes and transducers on the A&B sections for annular pressure monitoring. The production tree valves are open. The production fluid facility consists of 5 steel construction 400 barrel fluid capacity tanks situated inside a polymer lined steel containment dike area. Vegetation and erosion was observed on all outward facing slopes and all earthen stock pile areas. The well site meets ORC 1509.072.
