Annular pressure monitoring was in place for the 9.625 inch diameter casing gauge read 1225 pounds per square inch, the 13.375 inch diameter casing gauge read 0 pounds per square inch. Production equipment included a separator, Gas Processing unit, line heater, and 4- 400 barrel purge tanks shared with 3401320886, 3401320888. Pressure release lines had been installed on annular casings. Identification tags were present and production was up to date. |

The lease road and well pad surface are of stone surface construction and are in good repair. There is a 3rd party midstream operation along the lease road at the right front corner of the well pad. The production equipment consists of 3 gas processing units and a tank farm that is made up of 4 steel construction 400 barrel production tanks along with a 50 barrel steel tank all located inside a polymer lined steel containment dike area. There is signage posted at the entry point of the lease road and at each well head, hazard warning labels are posted on the tank battery. There is a gate across the lease road that is kept locked when no one is present on site. The well head is equipped with a lower B and upper A section with 2 diameter riser pipes present on the outlet ports on each section that are equipped with valves and gauges. The A section gauge that monitors the pressure between the 13.375 and 9.625 diameter casing strings reads zero pressure and the B section gauge which monitors the pressure between the 5.5 and 9.625 diameter strings reads 1,200 pounds pressure. The 5.5 diameter production casing well head section gauge reads 750 pounds pressure while the tubing head and flow line reads 150 pounds pressure. |
