0066462981 2015-06-30 Drill / Deepen / Reopen No Violations Current operations are assembling the 12.25 bottom hole assembly. Following reaching the total depth of the 12.25 open hole section the 9.625 casing will be ran and cemented.
0367338406 2015-04-28 Casing String Witnessed No Violations On this day the 30 conductor casing was ran and cemented into a 36 open hole to a depth of 100. Allied ran 417 sks of class A cement with cement to surface established. No violations noted.
0617398709 2018-06-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5 inch diameter production casing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625 inch diameter intermediate casing gauge read 325 pounds per square inch, the 13.375 inch diameter surface casing gauge read 260 pounds per square inch. Production equipment included a Sand Trap, vertical fluid separator, and heating coil. There are 2- 500 barrel stock tanks shared with 34013208930000, 34013208950000, 34013209380000. Identification tags were present, and production was up to date. No violations were found at the time of my inspection.
1047977982 2017-10-11 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains two 400-
1230870606 2015-07-06 Drill / Deepen / Reopen No Violations Nomac Rig # 80 is drilling the lateral section. Upon reaching total depth they will run their production casing.
1263114217 2019-02-21 Preliminary Restoration No Violations Preliminary restoration status check inspection found the well site meets ORC 1509.072 with vegetation and erosion control in place. The well pad perimeter is protected by an earthen berm with an 8 chain link fence on the perimeter berm. Signage is posted at the lease road entrance point and on each well head as well as the pad entrance. 4 wells are present on the pad with annular pressure monitoring in place on the well head A&B sections. The production fluid storage consists of (2) steel 500 barrel tanks situated inside a polymer lined steel containment dike area. 4 gas processing units are also present on location. A mid stream pipeline operator identified as Mark West has a station on the left rear corner of the well pad.
1432058896 2015-05-19 Casing String Witnessed No Violations On this day the 13.375 casing was ran and cemented in a 17.5 open hole to a depth of 2296. O-Tex was on site to cement the casing. See cement ticket for details. No violations noted.
1435775365 2015-07-01 Drill / Deepen / Reopen No Violations Nomac Rig # 80 has reached total depth in the 12.25 open hole section. They will be running 9.625 casing and cementing it with a one stage cement job.
1436194303 2015-07-02 Casing String Witnessed No Violations On this day the 9.625 casing was ran and cemented to a depth of 9160. Cement job produced 40 bbl of gel spacer back to surface. Calculated cement was just below surface. A bond log will be ran and evaluated to confirm top of cement.
