0135401291 | 2018-07-31 | Preliminary Restoration | No Violations | Preliminary restoration status check inspection found the area affected by the well pad construction has erosion control in place and vegetation established, the well pad surface and lease road are of stone construction and are in good repair. The entry point of the well pad is gated and locked with proper well site identification posted at the entry point of the lease road. There is an earthen berm established around the well pad and the company is controlling any collected surface water from rain events. There are a total of 5 wells on the pad that are in production at this time. The company has emergency shut down master control buttons and fire extinguishers place at all critical points on the well pad. The well head assembly consists of a lower B section and upper A section along with a production tree assembly. There are riser pipes with valves and manual pressure gauges attached to the A & B section that is allowing the company to monitor annular pressure. The pressure on the A section which monitors the pressure between the 13.375 diameter surface casing and 9.625 diameter intermediate casing string is currently 125 pounds per square inch (psi) and the pressure on the B section which is the pressure between the 5.5 diameter production casing and the 9.625 diameter intermediate casing is 750 pounds pressure. The 5.5 diameter production casing internal working pressure is 1,500 psi and the production flowline pressure is 1,000 psi. There is an emergency shut down master control device on the production flowline downstream from the production tree manual control valves. There is a total of 5 gas processing units on the well pad one for each well. On the back right hand corner of the well pad is the production fluid storage tank farm consisting of (4) steel construction 400 barrel tanks situated inside a polymer lined steel containment dike area. There is also a EQT midstream station next to the well pad entry point. |
0410828277 | 2016-01-04 | Drill / Deepen / Reopen | No Violations | On this day H&P Rig # 628 was in the second intermediate section of the well bore with a 12.25 drill bit. They were at a measured depth of 6645. They lacked approx. 2400 to reach their total depth. Once total depth is reached they will run and cement their 9.625 casing string. |

0712684957 | 2017-10-19 | Production Wells | No Violations | This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area that contains four 400-barrel production fluid holding tanks. Per signage, it appears that Rice Energy is conducting midstream operations on this site. Production records for this well are current to 2017 within RBDMS. |

1609624828 | 2016-01-06 | Drill / Deepen / Reopen | No Violations | On this day H&P Rig # 628 was approaching total depth of the 12.25 section of this well bore. The 9.625 casing is anticipated to be ran and cemented tomorrow. |
1836004021 | 2016-01-19 | Drill / Deepen / Reopen | No Violations | On my inspection today with MRI Andrew Corder, we found the H&P Rig 628 had reached total measured depth of 18,052 feet and was in the process of setting in place their 5.5 inch production casing. A walk-around inspection of the well pad indicated good placement and maintenance of secondary containment. |
