0563862282 2019-02-25 Preliminary Restoration No Violations Status check inspection found the well site meets ORC 1509.072 with vegetation and erosion control in place. The well pad and lease road surface is stone construction. There are 3 producing wells on the pad with the said well producing thorough a production tree with no artificial lift present. Proper well identification is posted at the well head. And hazard warning labels posted on the tank battery. The tank battery consists of (4) steel tanks capable of holding 400 barrel fluid volume each, all situated inside a polymer lined steel containment area. The well pad perimeter is protected by an earthen berm and in good repair. There is 3 gas processing units present on the well pad.
0628386850 2018-03-22 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 440 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 690 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . The production records within RBDMS are currently updated to 2017.

1469384038 2016-01-19 Drill / Deepen / Reopen No Violations On my inspection today with MRI Andrew Corder, we found Nomac Rig 83 drilling 8.5 inch wellbore at a depth of 10,739 feet, with a targeted total depth of 20,300 feet. A walk-around inspection of the well pad indicated good placement and maintenance of secondary containment, and the off-containment areas of the pad had recently been re-graveled.
2049176694 2016-01-07 Casing String Witnessed No Violations On this day Nomac Rig # 83 had reach a total depth of 396. The 18.625 casing was ran and cemented into a 24 open hole. Five bow spring centralizers were utilized to ensure proper standoff from the well bore. Stingray Cementing Company was on site to cement the casing with 590 sacks of Class H cement. Cement additives included 1.5% bentonite gel, 3% CaCl, and .25# flake per sack of Celluseal. Circulation of the well bore was achieved and cement to surface was established.
