0341344659 2016-06-04 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. Patterson Drilling drilled 475 of 24 borehole, then installed 450 of 20 casing. The casing sat down early and attempts to wash it down were unsuccessful. Halliburton cemented the casing in place with 390 sacks of BondCem at a 2.09 yield and 15.6 weight. I witnessed cement weight verification with pressurized scales. Mix water was tested for chlorides and ph before the job.
0386611550 2017-12-19 Production Wells No Violations On the day of my inspection I was accompanied by inspector Andrew Corder. We came to view the pad before the longwall panel had mined passed the pad on the southern side. This was done to see the site prior to mining. Gauges read in a clock wise rotation around the wellhead started at the 12 oclock position are as follows: 3500, 500, 0, 3600, 2800. Gauges are not labeled and cellar is full of river gravel. Production equipment includes three GPUs, three 400 barrel steel tanks, a line heater, and a vertical seperator.

1461182702 2016-06-07 Casing String Witnessed No Violations I am onsite to witness the first intermediate casing cementing operations. Patterson Drilling 296 drilled 2300 of 17.5 borehole. 2275 of 13.375 casing was then installed in the well. Haliburton cemented the casing in place with 1035 sacks of class A based BondCem. I verified the cement weight throughout the job by witnessing Haliburton personnel use a pressurized cement scale. I witnessed 80 barrels of cement circulate to surface. The casing integrity test was passed by increasing the pressure, after landing the plug at 1000 psi, to 1500 psi and holding for five minutes.
1468513616 2016-07-14 Casing String Witnessed No Violations I am onsite to witness the production casing installation operations. Patterson Drilling rig 296 is running 5.5 HPC-110 casing at a depth of 7,121. Solid body centralizers are being installed on every third joint. I reviewed the cement program for the up coming job tonight.
1551725185 2019-03-04 Production Wells No Violations Status check inspection found a total of (3) wells present on the pad with all 3 wells in production. All wells are equipped with annular pressure monitoring on the well head A&B sections, the pressure monitoring is being monitored through 2 diameter riser pipes with both manual gauges and transducers present. The well is selling off a production tree with 3,800 psi on the production tree and 1,200 psi on the flow line downstream from the production tree choke manifold, the 5.5 production casing pressure is 2,200 psi. On location is a total of (3) gas processing units. Also present is the production fluid storage area consisting of (3) steel construction 400 barrel production tanks situated inside a polymer lined steel containment dike area. Proper well identification is posted.
1657070084 2017-12-29 Production Wells No Violations On the day of my inspection I was accompanied by Engineer tech Gerry Smith. We came to view the pad for longwall mining subsidence on the southern side of the pad. No damage was noted of the fill slope on the eastern side of pad. Gauges read in a clock wise rotation around the wellhead started at the 12 oclock position are as follows: 3200, 500, 0, 3600, 3000. Gauges are not labeled and cellar is full of river gravel. Production equipment includes three GPUs, three 400 barrel steel tanks, a line heater, and a vertical seperator.
2103329640 2018-08-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 4-string casing design and was in normal production. Annular pressure monitoring was in place on both the 9.625-inch diameter casing gauge read 810 pounds per square inch, the 13.375-inch diameter casing gauge read 20 pounds per square inch. Production equipment included an individual Gas Processing unit, and 3- 400-barrel production fluid tanks shared with the other producing wells on the pad. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current.
