0530264749 2019-04-16 Preliminary Restoration No Violations Status check inspection accompanied by ODNR Inspector Rick Brown found the well site meets ORC 1509.072. 4 wells are present on the pad. The pad perimeter is in a fence in enclosure with a gate present. The company was also notified that the well API identification number indicated 3401321060-0000, I notified them that the correct number was 3401321063-0000.

The well is free flowing as it does not appear to have any artificial lift technology present at this time. The well is piped to a gas processing unit and then to a production area. The production area contains four 400-barrel production fluid holding tanks. The production records within RBDMS are current to 2017.

The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 50 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 1650 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . The production records within RBDMS are currently updated to 2017.
1010277772 2017-03-28 Urban Drill / Deepen / Reopen No Violations A meeting was held with the Frank Bartyzel the adjacent landowner of the well. Frank was concerned with the smell coming from the well site. Upon inspection of the well site, I utilized an H2S monitor to determine if there was any H2S present. My monitor did not have any readings of H2S on site. I contacted the company and ask them to come up with a plan to mitigate the foul smell. The well was equipped in a manner that made it appear to be capable of producing. The production line from the well went to a gas processing unit also on site. The tank battery area included four 400 barrel production fluid storage tanks. The production is then routed to a scrubbing unit, through another fluid separator, and then to the contact tower before being metered and going to sale.
1052234319 2016-10-16 Urban Drill / Deepen / Reopen No Violations Pioneer Rig 71 had reached total depth of the 24 open hole section of the well bore @ 368. A void was encountered @ 289-303. The 18.625 casing was ran to total depth with a packer set @ 236. O-Tex pumped a bottom plug to secure the shoe and then topped out cement to surface. Cement to surface was established.

1399924220 2018-07-31 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a three-string casing design with 5.5-inch diameter production casing and 2.875-inch diameter tubing that was open flowing. Annular pressure monitoring was in place for the 9.625-inch diameter intermediate casing gauge read 150 pounds per square inch, and the 13.375-inch diameter casing gauge read 190 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and 4- 400 barrel production fluid tanks, and a 50 barrel produced water tank shared with 34013210610000, 34013210620000, 34013210640000. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.
1468206411 2016-07-10 Casing String Witnessed No Violations 07/10/2016- On site at 21:00 hours to witness the conductor casing cement job. Butchs Rat hole top hole rig is on location and has drilled a 104 x 30 conductor borehole and ran 104 of 26 -.375 wall weld connection casing. O-Tex cement service cemented the casing using 260 sacks of class A cement with circulation established prior to cementing and no cement returns witnessed to surface with the fluid level dropping back down the well bore once displacement was complete. ODNR pre construction records indicated a historic surface mine was operated on the same property and the company will tag the top of cement and grout accordingly.
1477920026 2016-10-19 Urban Drill / Deepen / Reopen No Violations Pioneer Rig 71 had reached total depth of the 17.5 open hole section of the well bore @ 2086. The 13.375 casing was ran to total depth. O-Tex cementing was on site and cemented the casing with 1560 sacks of Class H cement. Cement to surface was established.
1493041503 2017-04-07 UrbanProduction Wells No Violations I received a call from Brent Bear to contact Greg Bieth concerning this well location. During my conversation with Greg, he expressed concerns with the well producing Hydrogen Sulfide gas. I informed him that I had checked the location previously and my detector did not show Hydrogen Sulfide present. The company has installed detectors to monitor the well for potential Hydrogen Sulfide and have not seen any as of now. I will do a follow up inspection to ensure that there is still no Hydrogen Sulfide present. Complaint # 5577.
1767794764 2017-04-07 UrbanProduction Wells No Violations Gulfport was contacted regarding the odor concerns coming from the well. I spoke with both Dave Lee and Judd Shreves. They stated they would work with Rice Energy who has a dehydration unit on site to mitigate the smell that they believe is a result of that unit. Complaint # 5577
1792615100 2018-08-22 UrbanPreliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with vegetation established and erosion control in place. The well pad perimeter is protected by a chain link fence with a gate present at the entry point of the well pad and along the lease road. Identification is present at both gates along with the entry point of the lease road. There is a midstream pipeline service point at the well pad entry point on the left side of the lease road. 4 wells are present on the well pad. The well head assembly consists of a lower A and upper B section with riser pipes present on the A&B section that is equipped with valves and gauges to monitor annular pressure. The A section which monitors the pressure between the 13.375 and 9.625 diameter casing strings shows 180 pounds per square inch (psi) on the gauge. The B section which is measuring the pressure between the 9.625 and 5.5 diameter casing strings shows 120 psi on the gauge. The 5.5 diameter casing string internal pressure is 1,800 psi and the production flow line pressure is 900 psi. The production tree consists of 3 manually operated valve and one emergency shut down valve. The well open flowing. Production equipment on location consists of 4 gas processing units along with a production fluid facility made up of 4 steel design tanks capable of 400 barrels fluid volume each tank. Emergency shut down control switches are present at all critical areas of the well pad.
1805497334 2016-10-24 Urban Drill / Deepen / Reopen No Violations A revisit to this site was conducted to follow up with complaint # 5459. The landowner alleges that the noise from the well site is disrupting his sleep. This is the last well to be drilled on the pad. I spoke with the company man on location and confirmed that this well is not being drilled any different from the previous wells on the pad. I will contact the landowner and discuss the complaint with him. I also contacted Gulfport to notify them of the complaint.
1865402170 2017-12-11 Production Wells No Violations I was on site to follow up on the fencing that Gulfport said they would provide around the well-head and tank battery. I contacted representative Blake Arthur to inquire on why the fencing has not been installed. Blake informed me that Gulfport plans to have the fencing installed by Christmas of this year. I will check back them to ensure compliance.
1931773880 2017-06-23 Production Wells No Violations During this inspection, I was accompanied by Brent Bear and Michael Moreland. We spoke to the well tender during our visit and discussed the wells production as well was the annular pressures noted on the backside. The annular pressure on the inside of the 9.625 inch diameter casing string was 1600 pounds per square inch. The surface casing string had 50 pounds per square inch noted. The production line from the well went to a gas processing unit also on site. The tank battery area included four 400 barrel production fluid storage tanks. The production is then routed to a scrubbing unit, through another fluid separator, and then to the contact tower before being metered and going to sale. Shane Ullom of Rice Energy stated that Rice takes over the gas sales starting at the scrubbing unit. This production from this well is being metered by both companies.
