0515675215 2018-08-23 Production Wells No Violations Status check inspection found the well pad perimeter protected by an 8 chain link fence with signage posted at the lease road and well pad entry points. The lease road and well pad surface are of stone surface construction. There are 5 producing wells on this pad and one well that was spudded and not complete. The said well is equipped with a lower A and upper B section with 2 diameter riser pipes equipped with valves and gauges attached to both sections to monitor annular pressures. The A section gauge which reads between the 9.625 and 13.375 diameter casing strings shows 20 pounds on the gauge. The B section which reads between the 5.5 and 9.625 diameter casing strings reads 490 pounds on the gauge. The 5.5 diameter casing internal pressure is 4,900 pounds pressure and the production flow line down stream of the choke is 1,700 pounds pressure. The well is equipped with production tubing and does have multiple surface safety and manual shut down valves. Addressing the production equipment present- there is 3 line heaters and 5 gas processing units on location. Also present is a production fluid storage area consisting of (2) steel construction 500 barrel fluid volume tanks situated inside a polymer lines steel containment dike area. The well pad is protected by an earthen berm and the drain valves are in the closed position.

1872518654 2018-09-27 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place the 9.625-inch diameter intermediate casing gauge read 500 pounds per square inch, the 13.375-inch diameter surface casing gauge read 0 pounds per square inch. Production equipment included a Sand Trap, and Gas Processing Unit. There were also 2- 500-barrel steel production fluid tanks inside secondary containment, that were shared with the rest of the producing wells on the pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was current in RBDMS.
