0084120105 2019-01-09 Production Wells No Violations Status check inspection found 4 wells present on the pad with all 4 wells being in production. All 4 wells are producing off the 5.5 diameter production casing through 2 primary master valves. The wells are still selling through sand trap separators with 2 500 barrel portable tanks present to service the sand trap separators when they are evacuated. There is a emergency shut down device installed in the production flowline at the well head. Annular pressure monitoring riser pipes are in place on the well head A&B sections, however no gauges are present. Additional production equipment is 4 gas processing units along with a production facility consisting of 4 steel tanks capable of holding 400 barrels fluid volume each.
0198715901 2018-07-18 Completion Testing No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that CWS snubbing rig was currently on location and tripping out of the hole. CWS had drilled out all isolation plugs and were at approximately 2700 feet with their 2.875-inch diameter tubing. After speaking with EQT company man Sheldon Zupper he informed me that they plan to continue to trip out of the hole and move the rig to 34013211220000. Sheldon also informed me that annular pressure monitoring was in place on all wells on the Pad.
0720493975 2017-09-06 Casing String Witnessed No Violations The rig ran 24 weld casing with all the required float equipment to 300. O-Tex Pumping cemented it with 435 sacks of Class H cement. 6 bbls of good cement was returned to the surface. The pressure test of the casing passed.
0740400521 2018-01-23 Completion Testing No Violations On the day of my inspection I was accompanied by Anthony Carson and Luke Saunders. We were on site to witness the construction of the above ground, open top, fluid holding tanks that the operator was installing to utilize during the completion phase of the wells. The operator had a service rig on site during our inspection.
0989705106 2018-07-18 Completion Testing No Violations Status check inspection accompanied by ODNR Inspector Ryan Armstrong. The well drilling and completion process is complete with a total of 70 stages stimulated. The completion plugs have been drilled out by a service rig snubbing unit combination owned by CWS which is rigged up on the #7 well on the location and drilling out the completion plugs.

1940284020 2017-09-07 Casing String Witnessed No Violations On location to witness the running and the cementing of the 18-5/8 mine string. The rig ran the casing with a shoe and baskets to 439. Otex balanced the bottom with 155 sacks of Class H. They also filled the annulus above the baskets with 445 sacks of Class H in 3 stages. 10 bbls of good cement was returned to the surface. A subsequent test of the casing passed.
