The well is equipped with (2) primary master valves used during the completion process that are in the open position, the valves are attached to the well head (B) section and are in the open position. Atop the master valves is a completion flow cross that is also equipped with a manually operated master valve followed by an remotely controlled emergency shut down valve. Attached to the flow cross is production flow back equipment with manually operated choke manifolds sitting in the area of the well heads controlling the outlet flow and pressure. There is a pressure valve on the flow cross that reads 5,000 (psi) pounds per square inch. On location is a total of 5 active wells all equipped in the same mannerism. Of the 5 wells only one well was equipped with one visual gauge on either of the (2) well heads sections where annular pressure can be monitored, with the (2) well head sections being the (A) section which is the 13.375 casing head section and the (B) section which is the 9.625 casing head section. All (5) cellars were standing full of fluid to surface. Situated on the well pad between the well heads and the production tank facility is a 500 barrel steel portable square tank being used to support the production flow back testing sand traps. It was observed that (2) of the outlet ports were unsecured with no cap of plug in place. The tank was inside a polymer liner that appeared to have recently overflowed with fluid, noted by the fluid level on the tank inside the liner. Attached to the top of the tank is a vent hose constructed of a crushable low pressure poly line, this line is attached at both ends with hose clamps and appears to be connected to a underground production system. Situated at the underground connection point of the tank vent is an electrical sump pump that is 110 volt (AC) alternating current design that is not explosion proof of vapor sealed. The e |
