0367575077 | 2017-05-02 | Casing String Witnessed | No Violations | I was on site accompanied by Inspector Bill Stolarik. Nomac 83 had reached a total depth of 1694 feet while drilling the 17.5 inch diameter open bore hole. The 13.375 inch diameter casing was ran into the open hole and cemented in place. Cement returns to surface were established. |

0802387950 | 2018-08-21 | Production Wells | No Violations | On the day of my inspection I witnessed that the well was equipped with a 3-string casing design with 5.5-inch diameter production casing that appeared to be open flowing with no artificial lift technology. Annular pressure monitoring was in place for the 9.625-inch diameter intermediate casing gauge read 100 pounds per square inch, and the 13.375-inch diameter casing gauge read 150 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and four 400-barrel production fluid tanks, and a 50-barrel produced water tank shared with the other producing wells on the well pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date. |
0945236810 | 2018-03-22 | Production Wells | No Violations | The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. There were also three 500-barrrel frac tanks on site. Annular pressures read on the gauges provided appeared to be 90 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 25 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . The production records within RBDMS are currently updated to 2017. |

1346259688 | 2018-08-07 | Production Wells | No Violations | Status check inspection found the well pad surface and lease road has a gravel surface and is in good repair. There are 6 wells present on the pad all in production. The said well is equipped with a lower A section and upper B section with 2 diameter riser pipes present at the well head and equipped with valves and manual pressure gauges to monitor annular pressure. The 13.375 x 9.625 diameter casing pressure was 100 pounds per square inch (psi) the 9.625 x 5.5 diameter casing pressure was 50 psi. The 5.5 diameter production casing internal working pressure was 1,800 psi and the production flow line pressure was 1,000 psi. The well is equipped with production tubing and was free flowing above line pressure. There are 6 gas processing units on location. At the right rear corner of the well pad is the production fluid facility consisting of 4 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment dike area. There is also a midstream station at the right front corner entry point of the well pad. Fire extinguishers and emergency shut down control points are present at all the critical areas of the well pad. The well pad perimeter is protected by an earthen berm. Also on this day I contacted Gulfports Production Superintendent Robert Coots and we discussed the 9.625 annular pressures on several of the wells on the pad along with having the well pad annulis monitored more closely while stimulation operations are going on the neighboring well pad. |
1779890485 | 2017-05-02 | Casing String Witnessed | No Violations | On site inspection to witness the surface casing cement job along with ODNR Inspector Andrew Corder. Upon arrival it was observed that Nomac Drilling rig # 83 was on location and rigged up on the hole with Stingray Cement Service standing by to cement the casing and Wyoming Casing power tong service rigged up and making up the pipe connections at 13,000 foot pound of torque per connection. Nomac Drilling has drilled a 17.5 diameter surface borehole to a total measured depth of 1,694 and ran 1,694 of 13.375 diameter J-55 Grade Buttress thread casing with a guide shoe ran on the bottom of the string and float collar set in the string at 1,650. Proper casing centralizer placement was demonstrated. The drilling rig loaded the well bore with 8.3 pound per gallon freshwater prior to pulling the drill string and circulated the last 5 casing joints in to bottom with no restriction. Stingray Cement Service cemented the casing using a 320 barrel lead cement mix consisting of 1500 sacks of Class A cement mix with 1% calcium and .25 pound per sack flow seal loss circulation material. Upon the pumping of the cement the plug was released from the cement head and the cement displace to the float collar with 253 barrels of fresh water and the casing was pressure tested to 1,182 psi achieving 500 psi over displacement pressure prior to the landing of the plug. The cement weight was verified by weight of scale 2 x during the job. Cement returns were witnessed to surface. |
