1498747274 2017-06-29 Casing String Witnessed No Violations On site inspection at 8:00 am to witness the surface mine string combination with Sidewinder Drilling rig 103 on location along with Halliburton Cement Service and Wyoming Casing Service. The rig has drilled a 24 diameter surface borehole to a measured depth of 648 off of a rig Kelly Bushing heigth of 30.5. A mine rubble seam was encountered at 562 to 574 with no circulation to surface past the mine while drilling or cementing. Upon reaching total depth drilled the rig pulled the drill string and ran 648 of 18.625 diameter J-55 Grade buttress thread 87.5 pound per foot casing with a float collar ran at 605 and a swell packer ran at 477, Halliburton Cemented the casing in a two step process with the 18.625 diameter casing tail section below the mine cemented 1st by pumping a 68 barrel class H cement mix displaced with 185 barrels of fresh water. The cement mix was verified by weight of scale. Proceeding the bottom balance job a 1 diameter tubing was inserted into the annular space and the annular space cemented from the packer to surface.

1795965387 2019-01-15 Production Wells No Violations Status check inspection found (3) wells total on this well pad with all 3 wells in production. All 3 wells are producing through a production tree through 3 manual gate valves and one emergency shut down valve with an electric choke present in line. The said well does have 2 diameter riser pipes present attached to the well head A&B sections outlet ports. The riser pipes are equipped with valves and gauges (both electronic transducers and manual gauges) the purpose of the gauges is to monitor annular pressure. The A section well head pressure is 320 (PSI) pounds per square inch, the B section pressure is 800 psi. The 5.5 diameter production casing pressure is 4,200 psi and the flowline pressure down stream from the choke is 1,600 psi. There is an emergency shut down control point at each of the 3 well head along with each critical point on the well pad. On site production equipment consists of (3) gas processing units along with a tank battery that contains (4) steel tanks capable of holding 400 barrels of fluid each along with a steel 50 barrel tank, all situated inside a polymer lined steel containment dike area. There is a third party (EQT) midstream pipeline station adjacent to the right front corner of the well pad. The lease road and well pad surface is stone construction and in good repair. The well pad perimeter is protected by an earthen berm. The Preliminary Restoration is due however there is snow on the ground during my inspection.
