0983366535 2017-08-22 Casing String Witnessed No Violations On the day of my inspection I witnessed the mine string cement job. Pioneer 71 drilled a 24 inch diameter open hole to a depth of 564 feet. Previous casing set and cemented was 26 inch diameter conductor casing to a depth of 117 feet. Wyoming caisng ran 564 feet of 18.625 inch diameter casing, j-55, 87.5 lbs/foot. Casing was centralizer with 3 centralizers. Mule shoe is 29.5 feet long. Cement Baskets were set at 373, 313, and 273 feet. Packer set at 313-333. Void encountered from 492 to 510 feet. O-tex was onsite with 740 sacks of class A cement. Additives include 1.5% CaCl, 0.6% C-16A, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 30% excess ran in open hole on bottom job. Cement displaced with 152 barrels of fresh water. No barrels of cement returned to surface. 15 barrels of cement pumped down backside to set up on packer. Backside was then loaded with 58 barrels of water. An additional 68 barrels of cement was pumped down backside through 1 inch diameter tubing after a 2 hour wait time. 10 barrel of cement returned to surface. Mix water has pH of 7, 46 ppm chlorides and 79 degrees Fahrenheit.

1500058820 2018-11-14 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. All erosion control filter sock material placed on the outward slopes during construction have been removed. The well pad and lease road are stone construction and in good repair. Signage is posted at the lease road entry point and at each well head with hazard warning labels on the tank battery. Emergency shut down devices and fire extinguishers are situated at each critical area of the well pad. There is a 3rd party mid stream operation along the lease road on the left side entering the well pad. The lease road does have a gate to restrict access. A total of 6 wells are present on the pad with the said well equipped with an A&B section well head along with a tubing head production tree. 2 diameter riser pipes with valves and gauges are present and attached to the well head A&B sections for the purpose of annular pressure monitoring. The A section gauge which is monitoring the 13.375 casing pressure reads 100 pounds per square inch pressure (psi) the B section gauge which monitors the 9.625 diameter casing pressure reads 80 pounds pressure. The 5.5 diameter casing pressure is 2,100 pounds pressure and the tubing head pressure before the choke valve is 1,800 pounds pressure. The production tubing well head valves are open to the sales line and an emergency shut down device is situated on the production well head tree. There are (6) gas processing units present on location and on the rear left hand corner of the well pad is the production fluid facility that consists of (4) steel construction 400 barrel fluid volume tanks along with a 50 barrel steel tank. All situated inside a polymer lined steel containment dike area.
1503648367 2017-08-25 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Pioneer 71 drilled a 17.5 inch diameter open hole to a depth of 2315 feet. Previous casing set and cemented was 18.625 inch diameter mine string to a depth of 564 feet. Wyoming casing ran 2312 feet of 13.375 inch diameter casing, j-55, 54.5 lbs/foot. Casing was centralized with 12 centralizers. Shoe track is 45 feet long. O-tex was onsite with 1666 sacks of class A cement. Additives include 2% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 30% excess ran in open hole. Cement displaced with 348 barrels of fresh water. 50 barrels of gel pumped ahead of cement. 51 barrels of cement returned to surface, and floats held with 2 barrels back. Mix water has pH of 7 and 38 ppm chlorides.
