1040570958 2018-02-20 Completion Testing No Violations On the day of my inspection the site was still being occupied by H&P Rig 628. Although the rig was on location it was laid down and await mobilization off site. The trailers where the employees stayed were still on location. The well was equipped with 5.5-inch diameter production casing and was shut in. The well is awaiting completion operations at this time.

1098744017 2017-11-22 Casing String Witnessed No Violations On site inspection to witness the 18.625 diameter casing repair. The casing parted during the initial cement job at a collar 10 above the inflatable swell packer. The initial well bore was 24 diameter drilled to 459 off of a 26.8 drilling rig Kelly Bushing heigth. The top of the Swell Packer including the 10 pup joint was at 346. A company identified a Wildcat Oil Tools manufactured a overshot casing patch to set down over the 18.625 casing collar looking up in the well bore. The casing was re assembled and the annular space topped off from surface by Stingray Cement Service with a 15.6 pound per gallon class A cement mix with initially 80 barrels of cement applied to the well with no returns to surface. The well bore was not holding fluid prior to cementing and a down hole camera was ran to verify casing condition and fluid level prior to running the casing.
1150682181 2017-08-28 Drill / Deepen / Reopen No Violations On the day of my inspection I found that HAD drilling has set 97 feet of 26 inch diameter casing by way of concentric drilling. Mouse hole was also concentrically drilled. Cellars are to be cemented tomorrow morning. Pad address is 57084 Nixon Run Road, Martins Ferry Ohio 43935. Spudded 8/25/17.
1287855241 2019-05-02 Completion Testing No Violations Status check inspection found 7 wells present on the well pad. All 7 wells have been drilled and completed and the completion plugs drilled out. The completion rig has finished up and is off location. All 7 wells are equipped with 2 primary master valves and will produce off the 5.5 diameter production casing. Flowback has not moved onto location and began rigging up yet.
1546469498 2019-01-02 Completion Testing No Violations On this day I attend the pre stimulation safety meeting along with all current contractors and company representatives. 7 wells are present on the pad, all well have the drilling process complete and are rigged up to be stimulated. The stimulation company is Stingray Pressure Pumping Service and the wireline is Renegade Wireline along with Select Energy Flowback. The well head A & B Sections are Cactus with the completion stack being from B&B. The well head B section which is the 5.5 x 9.625 annular space does have pressure control attached with electronic transducers in place to monitor pressure. Transducers are also attached to the A section which monitors between the 9.625 and 13.375 well head section. The well pad surface is of stone construction with earthen berms in place.
1573774251 2018-08-22 Completion Testing No Violations Status check inspection found the well drilling process is now complete. The well head configuration consists of a Cactus Company lower A section that is attached to the 13.375 diameter casing string, this is a new style well head system that the 9.625 and 5.5 diameter casing string land out in the A section by the means of mandrill hanger systems. The B section well head assembly consists of the 5.5 production casing flow cross only. The company has removed the lower and upper completion master valves and installed a service cap and 2 butterfly style valves to allow monitoring and pressure relief of the internal of the 5.5 diameter production casing which has not had the toe sleeves open or the initial set of perforations shot in the casing. A company identified as Full Circle is on site installing riser pipes on the well head A sections to monitor annular pressures. 7 wells are present on location and no production equipment is present on location. A mid stream pipeline is being installed to the pad midstream service point. 7 wells are present and have been drilled on the pad. The rig that drilled the wells identified as H&P 628 is disassembled and stacked out on location.
1911794160 2017-11-22 Casing String Witnessed No Violations On site status check inspection to witness the 18.625 diameter surface casing repair and cement job. The initial call out was for 22:00 hours on 11/21/2017 an upon arrival the company man indicated they incorrectly informed me and were at least 8 hours behind. I then left location.
2091729897 2019-03-20 Completion Testing No Violations Status check inspection found the well drilling and stimulation process complete on all 7 wells on the pad. Annular pressure monitoring is installed on each well on the pad with no pressures noted on any well above 500 psi. The tin horn cellars are free from standing fluid. A 3rd party contractor identified as shale pro was on site installing the surface production equipment, all sub surface equipment was previously installed. There are some areas of disturbance around the well pad perimeter where the mid stream operator installed the mid stream pipe line and the fresh water flow line was located during stimulation, these areas will need vegetation established. The company has a service rig on the neighboring Ridgetop pad and once drill out is complete there the rig will move over to the said well pad.
