0493252183 2017-12-11 Casing String Witnessed No Violations Onsite to do a hydrostatic for 13.325 inch diameter casing. Casing set at 785 feet and drilled new formation to 795 feet. Hydrostatic held for 30 minutes.
0767442965 2018-11-26 Preliminary Restoration No Violations Preliminary Restoration found the well site meets ORC 1509.072 with erosion control in place and vegetation established. The well pad and lease road surface is stone construction and the well pad has a perimeter fence present with signage posted. 3 wells are present on location and all 3 wells are constructed the same with annular pressure monitoring riser pipes are present on the well head A&B sections with valves and gauges on the riser pipes. The A section riser pipe gauge shows 140 pounds pressure on the 13.375 diameter casing string. The B section riser pipe gauge shows 1,400 pounds pressure on the 9.625 diameter casing string. The internal pressure on the 5.5 casing is 1,533 pound pressure and the production tubing flow cross shows 1,200 pounds pressure . The production flow cross does have an emergency shut down valve present. The well is producing into a gas processing unit with a single glycol heater present to service the 3 wells on location. The tank battery consists of (4) steel 500 barrel fluid volume tanks situated inside a polymer lines steel containment dike area and is situated on the well pad.

1403100256 2017-10-11 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 785 feet. Previous casing set concentrically was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming Casing ran 777 feet of 13.375 inch diameter casing, j-55, 6.1 lbs/foot. Casing was centralized with 5 centralizers. Shoe track is 40 feet long. O-tex was onsite with 935 sacks of class H cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 35 barrels of gel pumped and 162 barrels of cement. Pressure went up and cementers shut down. Checked cement head and pressured up again with no luck. Rigged down and plan to wait 12 hours before drilling out and running a CBL. Mix water has pH of 7, 46 ppm chlorides and 67 degrees Fahrenheit.
1508170365 2017-10-13 Casing String Witnessed No Violations On the day of my inspection I witnessed a top out job on this well. Surface casing cement job on 10/11/17 locked up. A CBL was ran. 200 of 1 inch diameter tubing was ran down the backside and cemented back to surface with 96 barrels. 14 barrels returned to surface.
1513716687 2017-12-19 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Pioneer 77 was drilling an 8.5 inch diameter hole at 11,204 feet. Last well on the pad. Rig mats are being sprayed off, possibly for upcoming rig move. Containment in this area was veiwed and is holding the overspray.
