Annular pressure monitoring is not in place on the well head A or B sections. |

The well pad and lease road surface is stone construction and in good repair. The said well has no annular pressure monitoring or identification posted and is being flowed through a vertical sand trap separator with the fluid being ran into a temporary storage area consisting of (2) 500 barrel horizontal tanks. There is 3 gas processing units present on the pad. The permanent production fluid storage area consists of (4) steel tanks capable of holding 400 barrels fluid volume each and one horizontal 100 barrel tank, all situated inside a polymer lined steel containment dike area. |

Production equipment included an individual Gas Processing unit and 4- 400-barrel production fluid tanks, and a 100-barrel production fluid tank shared with the other producing wells on the pad. There was a small leak on the 9.625-inch diameter casing, I spoke with EQT production superintendent John Allander and he informed me that they would send someone out to remediate the problem. Tim Hendershot arrived on location and evaluated the problem and dispatched a water truck to the pad to pump out cellars and remediate the leak. The well pad and all equipment were within earthen containment with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag were present, and production was current. |

Coal encountered from 407-413 feet. BJs was onsite with 871sacks of class A cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 155 barrels of fresh water. 35 barrels of gel pumped ahead of cement. 30 barrels of cement returned to surface. Floats held with 4 barrels back to truck. |

On Saturday 1/20, BJs pumped 1239 sacks class H lead cement and 731 sacks of class H tail cement and displaced with 55 barrels of fresh water when pressure spiked and job locked up. They ran a cement bond log and got permission from ODNR to top out with the 800 feet of 1 inch diameter tubing they ran on 1/20. Today, they established circulation through the 800 of tubin however the pressure was 900 psi pumping freshwater at .5 barrel per minute. The ran another string of 1 inch tubing to 480 feet and pumped 105 barrels of class A neat cement. 20 barrels of good cement returned to surface weighing 15 ppg. |

There are 2 wells on the pad that were previously drilled and completed identified as the Iron Warrior # 2 and # 4 wells. 3 new wells were drilled and are in the initial flow back production stages being produced through choke manifolds and sand trap separators, these wells are identified as the Iron Warrior # 1-3 & 5 wells. |
