0198421980 | 2018-08-14 | Completion Testing | No Violations | Status check inspection and documentation on the well drilling and completion process. The well drilling process is complete and the well is equipped with a lower A and upper B section with a production casing flow cross installed atop the B section and 2 valves installed vertically atop the production flow cross to allow the company to manage any unanticipated pressure that may be experienced on the 5.5 production casing while they are awaiting the well stimulation which is scheduled for January 5th 2019. The well head A&B sections are equipped with 2 diameter riser pipes, valves and gauges are present on the riser pipes to monitor annular pressure. I spoke with the company production foreman Eric Indermuhle on this day and they are monitoring and recording the pressures regularly. No production equipment has been installed on the well pad however the mid stream pipeline is being installed by EQT. The pad surface and lease road is of stone construction and in good repair. |
0285972094 | 2018-01-09 | Drill / Deepen / Reopen | No Violations | I conducted an inspection on this location during which I was witness to a hydrostatic casing shoe test on this well. The company drilled out the float equipment and continued to the shoe. The hole was loaded with water following the drill out. After a period of 30 minutes of monitoring it was determined that the hydrostatic test conducted was successful. The company will now continue the drilling of their 17.5-inch diameter open hole section of the well bore. |
1058327685 | 2019-03-20 | Preliminary Restoration | No Violations | Status check inspection found the well drilling and completion process complete on all 5 wells on the pad. A service rig owned by Hybrid Service Company is on location and has completed the completion drill out on the #5 well. Annular pressure monitoring is in place on each well head A&B section. 3rd party contractors are on site installing all surface production equipment. The preliminary restoration was reviewed and no erosion or vegetation issues were noted however some areas were disturbed around the pad during the installation of the midstream pipe line, the preliminary restoration will need followed back up on later after the disturbed areas have had a chance for vegetation to establish. The well pad and lease road surface are stone construction and in good repair. The well pad perimeter earthen berm is in good condition. |

1515363232 | 2018-01-07 | Casing String Witnessed | No Violations | On site inspection to witness the surface- mine string casing run and cement job. Pioneer Drilling rig 73 is on location and has mist drilled on air a 24 diameter borehole to a measured depth of 480 with the depth drilled off a 28.5 grilling rig Kelly Bushing heigth. The rig drilled through a mine rubble coal seam zone between the depths of 410 and 414. Upon reaching total depth the rig pulled the drill string and inserted 475 of 18.625 diameter J-55 Grade Buttress thread casing with a 42.7 casing shoe track ran consisting of a guide shoe and float collar and on casing joint between them. Also ran was a Halliburton Swell packer set between the depths of 325 and 329. Attempts to circulate the well bore prior to cementing were unsuccessful. Stingray cement service cemented the casing in a 2 stage process by first pumping a 25 barrel 6% gel mix followed by a 63 barrel absolute volume cement mix consisting of 298 sacks of class A cement with .3% calcium and .25 pounds per sack loss circulation material. The bottom balance cement job was pumped and displaced with 134 barrels of fresh water with circulation to surface saw at 70 barrels into displacement. Upon the landing of the plug casing pressure was increased to 530 PSI (pounds per square inch) and held for 5 minutes. Pressure was then increased to 800 psi with positive indications the packer had inflated, pressure was then bled off to 530 psi and then increased to 1,260 psi and held for 1 minute to closed the second set of valves inside the swell packer, the pressure was held for 60 seconds and at the point Stingray was preparing to release the casing pressure the casing jumped upward forcibly indicating a down hole casing failure. At that point Stingray pumped an additional 10 barrels of fresh water with no circulation or pump pressure and that point the casing crew was called back and the casing screwed back together with make up torque saw up to 7,000 pounds hydraulic pressure achieved. Fluid level was verified at |
