The well site boundary sedimentation control has been removed. The well pad and lease road surface is stone construction and in good repair. The well pad perimeter is protected by an earthen berm, there is also an 8 chain link fence present around the well pad. There are 5 wells present on the pad. Identification is posted on each well and production tank along with the entry point of the lease road and well pad. The well is equipped with a lower A section and upper B section with 2 diameter pipes present attached to the service ports of both sections for the purpose of annular pressure monitoring. The riser pipes are equipped with valves and both electronic transducers and manual gauges. The pressure on the well head A section is 451 pounds pressure and the pressure on the B section is 1,501 pounds pressure. The 5.5 diameter production casing well head flow cross pressure is 2,296 pounds pressure and the pressure on the production head flow cross section is 2,200 pounds pressure. The well is selling off the 5.5 diameter production casing through (2) primary master valves with a production tree installed on the master valves, 2 production flow lines are present, the pressure on the flow lines downstream from the pressure reducing choke valve is 1,350 pounds pressure and 1,186 pounds pressure. There are 5 gas processing units on location and (2) production fluid storage tanks situated inside a polymer lined steel containment dike area. There is a Mark West mid stream pipeline service point at the left rear corner of the well pad. Emergency shut down devices are present at each critical area of the well pad including the entry point of the location. |

Cement was displaced with 75 barrels of fresh water. No cement returned to surface. |

After speaking with Ascent Resources company man William Bowman, he informed me that they were currently pumping down the perforation gun for the 13th stage of the well. William Bowman also informed me that Ascent Resources was averaging 150 feet per stage and pumping 230 thousand pounds of sand per stage. I was following up from on an incident reported by Trisha Cartmell that there was a fire on the lease road at approximately 07:00 on 8/22/2018. I inspected the site of the incident and there was no contamination on the lease road. I witnessed a small amount of residue from the fire extinguisher deployment, William Bowman informed me that they had a contractor in route to location to remove the residue and replace the stone on the lease road. |
