0532367778 | 2019-02-07 | Preliminary Restoration | No Violations | Preliminary Restoration status check inspection found The well site meets ORC 1509.072 with vegetation established and erosion control in place. The well site boundary sedimentation control has been removed. The well pad and lease road surface is stone construction and in good repair. The well pad perimeter is protected by an earthen berm, there is also an 8 chain link fence present around the well pad. There are 5 wells present on the pad. Identification is posted on each well and production tank along with the entry point of the lease road and well pad. The well is equipped with a lower A section and upper B section with 2 diameter pipes present attached to the service ports of both sections for the purpose of annular pressure monitoring. The riser pipes are equipped with valves and both electronic transducers and manual gauges. The pressure on the well head A section is 421 pounds pressure and the pressure on the B section is 1,521 pounds pressure. The 5.5 diameter production casing well head flow cross pressure is 2,296 pounds pressure and the pressure on the production head flow cross section is 2,000 pounds pressure. The well is selling off the 5.5 diameter production casing through (2) primary master valves with a production tree installed on the master valves, 2 production flow lines are present, the pressure on the flow lines downstream from the pressure reducing choke valve is 1,422 pounds pressure and 1,337 pounds pressure. There are 5 gas processing units on location and (2) production fluid storage tanks situated inside a polymer lined steel containment dike area. There is a Mark West mid stream pipeline service point at the left rear corner of the well pad. Emergency shut down devices are present at each critical area of the well pad including the entry point of the location. |
1526858335 | 2018-05-20 | Casing String Witnessed | No Violations | On site inspection to witness the surface casing cement job with Patterson Drilling rig 795 on location along with Halliburton Cement Service. The rig drilled a 17.5 diameter surface borehole to a measured depth of 1,516 with the depth measured from a drilling rig Kelly Bushing heigth of 28.6 above ground level elevation. Upon reaching total depth the rig pulled the drill string and ran 1,515 of 13.375 diameter 61 pound per foot K-55 Grade Buttress thread casing with a casing guide shoe and float collar ran with casing centralizers on every 3rd casing joint. The last casing joint was circulated into the hole and circulation to surface was established at that point. Halliburton cemented the casing by pumping 50 barrels of 6% gel spacer followed by a single blend 306 barrel absolute volume primary lead cement mix consisting of 1,490 sacks of class H cement weighing 16.4 pounds per gallon by weight of scale with a yield of 1.151 and water ratio of 4.76 gallons per sack. The cement was displaced to the float collar with a total of 224 barrels of fresh water with cement returns witnessed to surface at 168 barrels into displacement. The final displacement pressure was 760 psi and the casing was pressure tested to 1,354 psi upon the landing of the plug. |
