The conductor weighed 94.62 pounds per foot. Water was encountered at 90-feet during the drilling process. During my inspection a pre-job safety meeting was held with all personnel on location and Petroset cementing rigged up and pumped 220 sacks of class A cement with 2 percent calcium chloride and displaced with 47 barrels of fresh water. Cement returns were established and were used to cement the cellar floor to create a liquid-tight cellar as per Ohio Administrative Code 1501:9-1-08 (F). |

Patterson 577 along with the assistance of Wyoming Casing Crew set the 18.625-inch diameter casing at 365 feet from the rig floor. Patterson 577 rig floor is at 25 feet from ground level. A mine void was encountered during the drilling process from 308-feet to 315-feet. A cement basket was set at 217. C&J Energy Services plumbed up to the 18.625-inch casing pressure tested their lines and pumped 127 sacks of cement. A cement sample was pulled and weighed in pressurized mud scales confirming the weight of 15.6 pounds per gallon. The plug was dropped and displaced with 104 barrels of displacement. After landing the plug the pressure was increased 500 pounds per square inch and held for 5 minutes. The floats held and .5 barrel was bled back to the truck. 1-inch diameter steel tubing was ran to 217 feet and C&J topped off the 22-inch borehole by 18.625-inch casing annulus with 201 sacks of cement. |

The cement was weighed in pressurized mud scales confirming the weight of 15.6 pounds per gallon. The plug was dropped, and 237 barrels were pumped for displacement. Estimated 13 barrels of cement returns established. After landing the plug the pressure was increased 500 pounds per square inch and held for 5 minutes. The floats held and .5 barrel was bled back to the truck. |

The Ascent Company man on location stated that the planned set depth was 18379 feet.
