0597534182 2016-03-29 Plug / Plug Back No Violations The Clark # 1 well was being prepared , by Rex Energy, for re-entry and re-plugging due to the close proximity to the Goebeler # 3Hs ( 34-019-2-2692, Washington Twp., Carroll Cty., R E Gas Development ) lateral wellbore. Beaver Excavating had cut and compacted the access road and location. The 11.75 casing stump had been dressed and a five (5) thousand psi flange welded on top. A blind flange ( night cap ) was bolted in place. I did not identify any violations during this inspection.
1191317710 2016-03-21 Plug / Plug Back No Violations I inspected the cellar construction at the Clark Unit # 1 on 3/21/2016. Rex Energy plans to re-enter the Clark Unit # 1 P&Ad wellbore and replug utilizing present day methods. This task is being undertaken by Rex, due to the close proximity to the lateral wellbore emanating from the Goebeler # 3H ( 34-019-2-2692, Harrison Twp., Carroll Cty., R E Gas Development ). Rex excavated around the casings and placed a six (6) X five (5) tin whistle around the casings and backfilled to construct the cellar. An expanded metal screen was placed over the cellar. The excavation revealed multiple casing strings starting with 24 conductor, 16 surface casing and 11.75 intermediate string. No violations identified during this inspection.
1403509531 2011-08-03 Final Restoration No Violations RBDMS - P/A 10-10-1969. Well location is now a crop field (corn). FR - passed.According to our database, this well was plugged on 10-10-1969. No visual evidence of well or production equipment. Site is a corn field. Final Restoration passed.
1531323793 2018-07-11 Final Restoration No Violations I conducted a final restoration inspection of this plugged well which was drilled-out and re-plugged by R.E. Gas Development in 2016 due to the proximity of the lateral wellbore to the Goebeler #3H well (34-019-2-2692). The well is located in a crop field which is currently being farmed and I located no surface expression of the well. Road mats were used during operations, so no lease road needed to be restored. Final restoration can be passed.
1631767919 2016-03-31 Plug / Plug Back No Violations I inspected the Clark #1 location to review the site construction prior to plugging activities commencing. Containment fabric had been placed over the entire location. Silt fencing had been positioned to control offsite erosion on the southwestern perimeter of the location. Newpark Services were on site placing rig matting on location and the access road. No violations identified during this inspection.
