None of the corrective measures prescribed by Inspector Murphy have taken place. The well is not equipped in order to establish commercial production levels ( 1509:062 ORC ). 0281887834 | 2015-08-21 | Administrative Inspection | No Violations | I inspected the Shirley # 3 as a follow up to a Notice of Violation ( # 1334673718 ) issued 4/17/2012. Martz Swabbing Service was swabbing on the 4.5 casing. I spoke with Mr. Martz, who said they started swabbing on 8/20/2015 but could not get deeper than approximately 4300 deep in the 4.5 casing. They loaded the casing with fresh water in case the blockage was a salt ring and it should be dissolved by the next day ( 8/21/2015 ). While I was on location, I observed Martz Swabbing personnel swab the 4.5 casing down to the obstruction at approximately 4300. They were pulling 90 of clear fluid at a time without any gas or fluid coming into the 4.5 casing. There was not any gas blow behind the swab or any oil on top of the fluid load. An impression block was ran, on the swab line, to the obstruction and removed from the 4.5 casing. An examination of the impression block did not reveal any markings on it. No interpetation could be made as to the nature of the blockage could be made. The Martz Swabbing crew shut down and were awaiting further orders when I left location. The violations detailed in NOV # 1334673718 have not been corrected at the time of my inspection. |

I received a call from the landowner during the week of 11/27/2017 stating that the crop field, in which the tank is located, was harvested and that the operator can proceed to move equipment off site. I spoke to Brian Levengood, of Dover Atwood Corp on 12/04/2017 and informed him of this, and also that the Chiefs Order cannot be closed until the final restoration is passed. He stated they will proceed to move all equipment off site as soon as possible. At the time of inspection, the production tank was still on site, so the final restoration cannot be passed at this time. |

The Division witnessed plugging of this well in December of 2016, and a plug report is on file. I located no surface expression of the well and vegetation was established. All production equipment has been removed. A landowner waiver was received by the Division for the electric poles and lease road. Final restoration can be passed. The compliance notice can be closed. |
0602732655 | 2015-02-10 | Production Wells | Violations Noted | I received a call from a landowner, Mr. Dean Scharver, concerning an idle well located on his property. He said he was told by someone that the well would be plugged and would like it done prior to planting season. I told him I would look into it and inspect the well. My research found that Inspector Shawn Murphy issued a Notice of Violation to Dover Atwood for non-production and failure to properly identify the well on 4/17/2012. I inspected the well and found that the rods and tubing have been removed as well as the pumping unit. The 4.5 wellhead is equipped with a 4X2 reducer and a 2 ball valve. There is an old, disconnected service box on a power pole and there is a metal belt guard lying in the weeds. The tank battery contains 2-210 barrel tanks, one of which is on its side, at an angle, and partially buried. The ID sign on the tank indicates the owner and an emergency contact number, but does identify the well information as required by law. There is no separation equipment on location that would allow this well to be properly produced. I later learned that Mr. Scharver had spoken with Inspector Bob Roush and that the issue is an open complaint on file with the Division. Inspector Roush is following up with Dover Atwood to resolve the violation. This violation continues. |

Inspection revealed that the well appears to be idle at this time. The pumping unit has been removed and the rods have been pulled from the well. All the ball valves on the well head are closed and rusted. Mr Sharver advised me that they werent getting any domestic gas from the well and he wants the operator to plug or produce the well. He advised that he isnt sure who the owner/operator is at the present time because Dick Fornash was the owner/operator but he passed away a few years ago. I advised him that when I figured out who the current owner is, then I will take the appropriate action and give the operator the choice of either plugging or producing the well. I checked the production records in RBDMS and found that the last production reported on this well was in 2001. |

Dover Atwood has owned the well since 2002, but there has been no production for the well during that time. Production for 2014 has not been reported at all. The violation continues. None of the corrective measures prescribed by Inspector Murphy have taken place. The well is still idle and incapable of production. The 4.5 casing has a 4.5 to 2 swage with a ball valve on top set in a closed position. The flowline is not connect to the tank battery. Nor is there a separator, pump jack, rods, tubing or electric lines present to establish commercial minimum levels ( 1509:062 ORC ) of oil and gas production. 1440611933 | 2015-08-14 | Administrative Inspection | No Violations | I inspected the Shirley # 3 location as a follow up to a Notice of Compliance ( # 1334673718 ) issued on 4/17/2012 for non-production. The original 210 barrel oil tank had been replaced ( due to integrity issues ) with a refurbished 210 barrel oil tank. An above ground two ( 2 ) inch flow line connected the storage tank and the well head. I did not see any evidence of a swab truck being on location until I passed a swab truck on Stallion Road heading to the Shirley # 3. The swab truck was from Martz Swabbing Services. I spoke with Mr. Martz who indicated they were employed by Dover Atwood to swab on the well. I told him that I would stop back in a couple hours to see the results of swabbing. Upon my return the swab rig was rigged upon on location, but no personnel. I inspected the well head and found that the two ( 2 ) inch ball valve had its stem broken and could not be opened. No corrections to the previous Notice of Violation had been performed. |
1481295970 | 2016-12-07 | Plug / Plug Back | No Violations | Nicks Well Plugging (NWP) moved on and rigged up over the Shirley # 3 well. The wellhead of the Shirley # 3 well was comprised of 4.5 casing with a 4.5 to 2 swage with a 2 ball valve on top. NWP ran in the hole with their ripping tool ( without the knife ) in order to tag the top of the obstruction. Using a depthometer the fluid level was found at approximately 850 down and the obstruction was found at 4293. After removing their ripping tool from the 4.5 casing NWP ran an wax impression ( 3.75 diameter X 1.25 depth ) block and tagged the obstruction. A surface examination of the impression block face was inconclusive. Operator decided to lower a 3.5 tricone bit, on the 2.375 work string, to the obstructions depth and attempt to drillout the blockage. The work string was plugged with paraffin and metal scale. After running approximately 30 joints the operation was halted and the well was secured for the night. No violations were identified. |

Due to paraffin and scale in the tubing only five joints of tubing had been ran prior to my inspection. Brian Levengood ( Dover Atwood ) decided to steam the tubing in order to make suitable for drilling. NWP removed the previously ran tubing and bit. A pit was dug and lined and the tubing was cleaned. No violations identified during this inspection. |

The well head was not connected to any flow line. The disconnected flow line consisted of plastic coated two (2) inch line steel pipe connected directly to 210 barrel storage tank. There was not any separation equipment present. The required I. D. signage was affixed to the tank. The access road was through a corn field and did not appear to have any recent uasage. The well was not capable of production at the time of my inspection. No corrective measures of the prescribed in the Notice of Violation ( # 1334673718, issued 4/17/2012 have been corrected |
