0838212500 | 2018-10-30 | Production Wells | No Violations | I was on site in response to complaint #5982, in which the complainant states the well leaks water during pumping operations. The pumping unit was not operating at the time of inspection, and I detected no visible leaking at the time of inspection. |
0906368299 | 2018-10-31 | Production Wells | No Violations | I was on site in response to complaint #5982, in which the complainant states the well leaks water during pumping operations which affects his domestic gas supply. The 8-5/8 casing was exposed and the unit was pumping at the time of inspection. I sat on site for one hour while the unit was pumping, and witnessed no leaks at the time. |
0949081910 | 2017-11-20 | Production Wells | No Violations | The well is equipped with a pumping unit and the production valves were in the open position. A compliance notice issued on 04/20/2015 was open pending proper identification at the wellhead. Correct identification was posted at the time of inspection. No violations were observed at the well or tank battery. The compliance notice can be closed. |
1436465587 | 2012-05-31 | Production Wells | No Violations | My inspection found the well has reported production for 2010 and no violaitons are noted at the well head or tank battery at this time. |
1463409366 | 2016-05-16 | Production Wells | No Violations | I conducted a compliance notice follow-up on the well. The well is equipped with an electrically-driven pumpjack and was producing at the time of inspection. No leaks were detected at the well. Current identification listing G&O as the owner with an updated emergency phone number is not posted at the well. The compliance notice will stay open until proper identification is posted at the well. |
1479469142 | 2018-10-14 | Production Wells | No Violations | I was on site in response to complaint #5982, in which the complainant states the well leaks water during pumping operations. The pumping unit was not operating at the time of inspection. 1543589638 | 2018-11-29 | Production Wells | No Violations | I was on location in response to complaint #5982 in which the complainant states the well leaks which affects his domestic connection. The 4.5 casing was still exposed as per my request to the operator, and I detected no evidence of leaks at the time of inspection. I tested the standing water around the well head using a low range QuanTab which concluded in a reading of 326 ppm. I spoke to the complainant, Jon Ward, by phone on 11/30/2018 and informed him that this complaint will be closed as I have not detected any evidence of leaking from the well and have found no violations. I requested that he contact me in the future if he witnesses any issues/leaking at the well. |
1588006722 | 2015-03-31 | Production Wells | Violations Noted | I was on location to conduct a status check of this well with DOGRM Inspectors Heidi Scott and Ashley Raff. The well is equipped with a pump jack and is in production. The valves on the production line are cracked open. The well has a domestic line and valves are open. There was a 3 foot diameter stain of old oil around the 8 5/8 casing. The belt guard for the pump jack is laying on the ground. The tank battery has one 210 BBL production tank and one production tank cut in half, along with a mechanical separator. The 210 BBL production tank valve does have a bull plug on the tank valve. Another production tank that had been cut in half was open at the top and full of residual oil and water. Fallen trees and vegetation were on top of the oil/ water filled tank and the tank had been disconnected from the other production equipment and was outside of the tank battery. A gas odor was detected around the tank battery. The dike around the tank battery is eroding along the east side and is directly adjacent to a stream. The signage on the tank battery was very poor and needs replaced. Review of records in RBDMS indicates the last reported commercial production was in 2012 for this well. The lease road is stable and drivable. The following needs completed by:
1) Remove the oil/water filled open half tank and clean-up oil around the wellhead (4-28-15) 2) A new identification sign needs posted (4-28-15) 3) Repair dike (4-28-15) 4) Remove unused pipe and equipment (4-28-15) 5) Reinstall belt guard on the pump jack (4-28-15) |
1727000924 | 2018-10-22 | Production Wells | No Violations | I was on site in response to complaint #5982, in which the complainant states the well leaks water during pumping operations which affects his domestic gas supply. The operator had excavated around the well head to expose the 8.625 casing and 4.5 casing and turned the unit on to cycle. I witnessed operations for 2.5 hours during the pumping cycle. The domestic connection was equipped with a gauge, which displayed 80 psi after approximately one hour, and then held steady at 40 psi. I did not locate or witness any leaks before or during pumping operations. I located no violations at the well at this time, but will keep the complaint open pending further investigation. |
1938023649 | 2018-10-18 | Production Wells | No Violations | I was on site in response to complaint #5982, in which the complainant states the well leaks water during pumping operations. The pumping unit was not operating at the time of inspection. I spoke with the operator, who will be meeting with me in the near future to excavate the area around the well head so that I can observe operations while the unit is operating. |
2023057916 | 2015-06-23 | Production Wells | No Violations | A belt guard was installed on the pumpjack, and proper identification was posted. Unused pipe and equipment including the oil/water filled open half tank had been removed. The tank battery now consist of one, 210 barrel production tank which was shared with PN#2430. The tank battery has been moved south so that is now located further from the stream. A dike has been constructed around the tank battery. Proper identification has been posted at the tank battery. |
