1111097129 2015-10-15 Production Wells Violations Noted I was on site with DOGRM Regional Supervisor Tom Hill to conduct a status check of this well. The well was equipped with an electrically-driven pump jack. The production valves were in the closed position. There was a temporary tank on a skid located approximately 12 feet from the well. The last reported production for this well was 25 bbls in 2012. A plugging permit has been issued for this well with an expiration date of 06/03/2016. In the interim, the temporary production tank needs to be removed from site.
1449851145 2015-12-11 Plug / Plug Back No Violations I was on location to witness the plugging of this Clinton Sandstone well. Plugging operations were conducted by William S. Miller Inc. (WSM) and Formation Cementing. The bottom plug was set on 12/09/2015 and witnessed by DOGRM Inspector Ashley Raff. The 9.5lb/ft., 4.5 production casing was ripped at a depth of 3958 and pulled from the hole on 12/10/2015. On 12/11/2015, tubing was spotted at 3730 and it was noticed that the conductor pipe (no record) was moving and had annular space. It was decided to fill the annular space with cement to prevent the possibility of oil or brine from entering the subsurface. The annulus was topped off 4.6 barrels (19 sacks) of slurry and held at the surface. Formation rigged up to the 4.6lb/ft., 2.375, regular tubing and mixed and pumped 18 barrels (75 sacks) of Class A cement slurry with 2% bentonite gel and 80lbs of cottonseed hulls. The slurry was displaced to 3430 with 13 barrels of freshwater. The tubing pulled wet and was flushed clear after pulling 20 joints. WSM pulled tubing to 1320 and Formation rigged up to the tubing. They then mixed and pumped 13 barrels (52 sacks) of Class A cement slurry with 40lbs of cottonseed hulls and displaced the slurry to 1111 with 4 barrels of freshwater. WSM pulled tubing to 211 and Formation rigged up to the tubing. They then mixed and pumped 18 barrels (74 sacks) of Class A cement slurry and pumped cement until good returns were achieved at the surface. The cement was holding at the top of the hole one (1) hour following the job. The top of the cement will be verified next week and the casing will be cut off and fitted with an ID plate. No violations.
1882524547 2015-09-16 Production Wells Violations Noted Inspection of the well showed the well is equipped with a pump jack. The tubing is shut. There is not a production tank affiliated with this well. A steel tank is 15 from the well head. Division records show that production has not been shown for this well since 2010. There is a plugging permit issued for this well that expires 06/03/2016. Supervisor Hill spoke with Bill Miller Jr. who informed Mr. Hill that this well is on their plugging schedule.
