0174038821 | 2015-03-24 | Plug / Plug Back | No Violations | I was on location at 2:15pm and Nicks Well Plugging (NWP) had fished 3000 of tubing from the hole and encountered pumping rods. They recovered 900 of pumping rod and the last piece was sheared off and corkscrewed. They ran another impression block and sat down on the sheared off rod. They will need to get an insert for the 2.875 overshot to retrieve the rods. Plugging operations continue. No violations. |
0796316521 | 2015-03-27 | Plug / Plug Back | No Violations | I was on location to witness the setting of the bottom plug across the Clinton Sandstone. Nicks Well Plugging (NWP) recovered 5,300 of 1.5 tubing and were unable to fish what remained. It was decided that we could pump past the remainder of the tubing and set the bottom plug. NWP rigged up to the 4.5 casing and then mixed and pumped 6.3 barrels of cement slurry with 50lbs of cottonseed hulls into the hole. The slurry was bullheaded to the perforations with 82.2 barrels of water. The plug began to land and locked up at 2000psi. The valve on the pump truck was closed and left shut-in for the weekend. No violations. |
1258874626 | 2015-02-17 | Administrative Inspection | No Violations | I received a phone call from Nick with Nicks Well Pluggers wanting some information on the above well. He had very little information. He was able to give me a name of the land owner and that the well was purchased from Petrox in 2003. With this information I was able to find it in RBDMS. There is a transfer of ownership on file. I emailed him all of the information He will be meeting with a represented for the landowner to apply for a plugging permit. I will follow up with an inspection report. |

1430153066 | 2015-04-02 | Plug / Plug Back | No Violations | I inspected this location to check the condition of the site following plugging operations on 3/30/15. All equipment has been removed from location and the circulating pit has been closed. The area has been graded to its original contour. I will conduct a final restoration inspection next month. No violations. |
1465222911 | 2016-06-06 | Final Restoration | No Violations | I conducted a final restoration inspection. The well has been plugged, and a plugging record is on file. All production equipment has been removed. The area has been graded and the roadway has been restored. Vegetation has been established. Final restoration can be passed. |
1738452191 | 2015-03-19 | Plug / Plug Back | No Violations | I was on location to witness plugging operations. Nicks Well Plugging (NWP) moved in on 3/18/15 and notified me that the top of the 1.5 tubing was 30 down the 4.5 casing and they will need to get fishing tools to pull it from the hole. I was on location 3/19/15 and NWP ran in the hole and hooked onto the tubing with a fish. The rig pulled 22,000 pounds on the string (7,000lbs over string weight) and the fish came off the top of the tubing. They ran an impression block and it showed the top of the tubing was crushed. They will bring in 2.875 tubing to overshoot the 1.5 tubing with a ripping tool. No violations. |
1945487488 | 2015-03-25 | Plug / Plug Back | No Violations | I was on location today to check the progress of fishing rods and tubing out of the hole. Nicks Well Plugging continued to rip rods and tubing out of the hole. It will take one more day to recover the tubing. No violations. |
2079301475 | 2015-03-30 | Plug / Plug Back | No Violations | I was on location to witness the plugging of this Clinton Sandstone well. Nicks Well Plugging (NWP) ripped the 4.5 casing at a depth of 4,075 and they pulled 8 joints to set a plug across the top of the Big Lime. They pumped 100 barrels of water to break circulation and then mixed and pumped 18 barrels of cement slurry. They displaced the slurry to 3,548 with 56.4 barrels of water. NWP pulled 76 joints of casing to a depth of 1,245, rigged up, broke circulation, and then mixed and pumped 12.8 barrels of cement slurry. The slurry was displaced to 1,045 with 16.5 barrels of freshwater. NWP pulled 32 joints to set the final plug in the well. They rigged up, established circulation, and then mixed and pumped cement until good returns were achieved at the surface (12.8 barrels). The remaining 7 joints were pulled from the hole and the 8.625 surface casing was topped off with 1.5 barrels of cement. 123 joints of 10.5lb/ft., 4.5 casing was recovered during the job. The cement was holding in the surface casing after 30 minutes and I left location. No violations. |
