0129311285 2015-01-14 Production Wells No Violations I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 8, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. Lightening protection is installed on top of the production tanks. The well is properly identified with a clear, legibly marked well identification sign. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. The lease road is stable and drivable. No violations found.

0783271591 2015-09-14 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0790399961 2016-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was open at the time of inspection. The #6H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence. The man doors were unlocked, but Chesapeake personnel were on site at the time of inspection.

The equipment on the pad consists of the following: 8- 400 bbl production tanks, three labelled as containing produced water, and five labelled as containing condensate 1 low-pressure vertical separator 7 Producing wells 7 Sand separators 7 Gas Processing Units 1 Heat Treater 1 Vapor Recovery Unit

1117979741 2018-02-26 Production Wells No Violations I conducted a status check of the pad. No changes were made to the operating equipment since my last inspection. The well is equipped with a plunger lift unit and the surface-intermediate annular pressure gauge displayed a reading of zero psi. No violations were observed at the time of inspection.

The access road was equipped with a gate and was locked. The #6H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 7 producing wells

1 Low pressure vertical separator 7 sand separators 7 gas processing units 1 vapor recovery unit 1 heat treater 1 compressor station
