0328061497 2015-11-30 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1150188306 2018-12-18 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate and intermediate/production annular gauges displayed readings of zero psi. I performed a walk-around inspection of the production pad and observed no erosional issues. No violations were observed at the well or production pad at the time of inspection.
1173434531 2012-07-30 Preliminary Restoration No Violations I was on site to check the status of the well. My inspection revealed that the well has been fracked and is waiting to be put into production. There is no surface operating equipment assembled on site at this time. My inspection also revealed that the Preliminary Restoration can be approved at this time.
1459949579 2016-03-10 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was equipped with a lock, but the gate was unlocked at the time of inspection. The #1H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. There was no standing water inside of the dike at the time of inspection. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 8 -400 bbl production tanks, five labelled as containing condensate, and three labelled as containing produced water 1 low-pressure vertical separator located outside of the dike 6 Producing wells 6 Sand separators 6 Gas Processing Units 1 Heat Treater 1 Vapor Recovery Unit

1862945289 2012-05-16 Drill / Deepen / Reopen No Violations Division of Oil and Gas Resource Management employee Kyle Tharp accompanied me during this inspection. Our inspection revealed that Nomac Rig# 311 is finishing rigging up and getting ready to spud the well. They should be ready to run and cement the 13 3/8 surface casing late tonight or early tomorrow morning. Our inspection revealed no violations at this time.
