0660070157 2012-08-23 Drill / Deepen / Reopen No Violations I was on site to check the status of Patterson Drilling Rig # 329. My inspection revealed that the rig is in the process of rigging down so they can walk the rig over to the Slates 1H well. I also obtained the 5 1/2 production casing/cementing information. The job was completed this morning at approximately 06:17am. Schlumberger cemented 13639 of 5 1/2 production casing with 2186 sacks of ClassA with 1.50% D020; .2% D046; .5% D207;.15% D153; .3% D201; and .1% D065. %1. Fifty one centralizers were installed on the casing string. No violations were noted during this inspection.
0876359110 2015-12-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1345521774 2012-08-10 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. My inspection revealed that Patterson Rig # 329 has drilled the 17.5 hole to a depth of 620 and ran 618 of 13 3/8 casing with five centralizers installed on the casing string. Schlumberger cemented the casing with 574 sacks of ClassA cement (2% D46; .125% D130; 2% S001) with a density of 15.6 pounds per gallon and a yield of 1.2 cubic feet per sack. There was good circulation throughout the job and approximately 35 barrels of cement was circulated to surface. My inspection revealed no violations at this time.
1684864056 2012-07-12 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 conductor casing. My inspection revealed that Express Energy Services drilled the 26 hole to a total depth of 112 and ran 112 of new 20, 94 pound per foot conductor casing. Allied Oil and Gas Services cemented the 20 casing with 277 sacks of Class A cement (2% CaCl and .25 #/sack Floseal) and circulated approximately 10 barrels of cement to surface.
1745571671 2012-08-13 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 intermediate casing. My inspection revealed that Patterson Drilling Rig # 329 has drilled the 12.25 hole to a depth of 2150. The drilling rig and Wyoming Casing ran the 9 5/8 casing to a depth of 2135. Schlumberger cemented the casing with 740 sacks of ClassA cement (10% DO44 (salt);.2% DO46;.15% D153; and .125 pound per sack D130) with a density of 15.6 pounds per gallon and a yield of 1.24 cubic feet per sack. There was good circulation throughout the job and approximately 36 barrels of cement was circulated to surface. My inspection revealed no violations at this time.
