0477350653 2012-12-12 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 intermediate casing. Patterson Rig # 329 ran 2094 of 9 5/8 casing (40 pound per foot) with 16 centralizers installed on the casing string (on joints 1,3,6,9,12,15,18,21,24,27,30,33,36,39,42,45). Schlumberger cemented the casing with 740 sacks of Class A cement with 10% salt; .2% D046, .15% D135, and .125 pound per sack D130 (flake). Schlumberger pumped 25 barrel gel spacer followed by 10 barrels of freshwater. They pumped 163 barrels of cement and displaced with 153 barrels of freshwater. There was good circulation throughout the entire job and approximately 14 barrels of cement was circulated to surface. They bumped the plug at 17:10 hrs at 900 PSI and conducted the casing integrity test at 1580 PSI ; test approved. My inspection revealed no violations at this time.
1056780928 2015-12-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1355124792 2012-12-10 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. My inspection revealed that Patterson Rig # 329 has drilled a 17 1/2 hole to a total depth of 639. They ran 629 of 13 3/8 surface casing with 6 centralizers installed on the casing string. Schlumberger cemented casing with 576 sacks of Class A cement with 2% CaCl (S001) ; .125 pound per sack D130 (flake); and .02 gallon per sack D047. They displaced the plug with 91 barrels of freshwater and bumped the plug with 380 psi at 4:40am. Pressure tested the casing at 975 psi with no loss of pressure; test approved. Inspection of the well site revealed no violations at this time.
