0423397480 2014-06-12 Casing String Witnessed No Violations I was on location to witness the cementing of the 13.375 surface casing. Nomac #72 drilled a 17.5 hole to a depth of 838 and Wyoming Casing landed the surface pipe at a depth of 828. Schlumberger cemented the casing with 158 barrels of Class A cement slurry weighing 15.6lbs/gal. 45 barrels of cement circulated to surface during the job. See cement report for more details on this job. No violations.
0647927322 2015-09-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of two 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:


Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1735214294 2014-06-13 Casing String Witnessed No Violations I was on location to witness the cementing of the 9.625 intermediate casing. Nomac #72 drilled a 12.25 hole to a total depth of 2,025 and Wyoming Casings crew was currently rigging up to run 2,013.5 of new, 40lb/ft., J-55 casing into the hole. Schlumberger cemented the casing with 149 barrels of SurfaceSET-b weighing 15.6lbs/gal. 14.5 barrels of cement circulated to surface during the job. See cement report for more details on this job. 1769234383 2017-09-06 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #8H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 2- 400 bbl production tanks; both labelled as containing produced water. 4 Producing wells 4 Sand separators 4 Gas processing units
