0119801142 2015-12-07 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:


Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1317716049 2014-06-21 Drill / Deepen / Reopen No Violations I was on location for the cementing of the intermediate casing. Nomac drilled a 12.25 Hole #3 to 1,760 from rig floor (RF) and set 9.625 casing to 1,750 RF with 10 centralizers. The casing had a weight of 40 lbs/ft and rating of J-55. A float shoe and collar was installed on the casing. The float collar was between the second and third joints. O-Tex rigged up and established circulation with 5 bbls of fresh water. O-Tex mixed and pumped 35 bbls of a gel spacer, 10 bbls of a fresh water spacer and 155 bbls (720 sacks) of Premium H cement with a yield of 1.21 and 15.6 PPG. Next, O-Tex released the plug and displaced the cement to 1,661 with 126 bbls of fresh water. I witnessed 37 bbls of good cement returned to surface. Once the plug landed, O-Tex pressured up from 1,000 psi to 1,490 psi and held static for 5 minutes. Pressure on the plug was release and there was 0.75 bbl of flow back water.
1403514973 2014-06-19 Drill / Deepen / Reopen No Violations I was on location for the cementing of the surface casing. I found during my inspection Nomac Rig 70 running the surface casing into the hole. O-Tex was on location to perform the cement job. Nomac drilled a 17.5 Hole #2 to 794 from rig floor (KB) and set 13.375 casing to 784 KB with 7 centralizers. The rig floor height was 2011. The casing had a weight of 54.5 lbs/ft and rating of J-55. I observed a guide shoe and float collar installed on the first joint of the casing string. Once the casing was set; O-Tex rigged up and established circulation with 5 bbls of fresh water. O-Tex mixed and pumped 30 bbls of a gel spacer, 10 bbls of a fresh water spacer and 149 bbls (700 sacks) of Premium H cement with a yield of 1.2 and 15.6 PPG. Next, O-Tex released the plug and displaced the cement to 741 with 114 bbls of fresh water. I witnessed 29 bbls of good cement returned to surface. Once the plug landed, O-Tex pressured up to 825 psi and held static for 5 minutes. Pressure on the plug was release and there was 1 bbl of flow back water. Cement samples was taken and weighed at 15.3 and 15.6 respectively. NOTE: Setting the casing took longer than expected. Nomac had to wash the casing into the hole due to buoyancy in the hole.
1410957406 2014-06-18 Drill / Deepen / Reopen No Violations I performed a status check on the above drilling location. Nomac Rig 70 is drilling a 17.5 hole to a purposed depth of 785. They should be at there purposed depth on 6/19/2014. I instructed them that I would be on vacation at that time and for them to call Inspector Rick Skidmore when they were ready to cement the 13 3/8 surface casing. I walked the location there is no spills, location is clean. No noted violations at this time.
1489485715 2016-02-25 Production Wells No Violations I was on location for a status check. My inspection found the access road gate locked and the access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #8H well is in production. The well is equipped with a christmas tree valve assembly. There is a steel dike around the storage tank.

The equipment on the pad consists of the following: 6 Storage tanks 1 Vapor recovery unit (VRU) 1 Gas processing unit (GPU) 1 Vertical separator 1 Sand separator 1 Thermal electric generator 1 Producing well 1 Conductor Site fenced and man doors locked
