0119844517 2015-09-28 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0437751050 2017-12-12 Production Wells No Violations I was on location to conduct a status check of this well. The pad is equipped with six producing wells, and this well was equipped with a plunger lift assembly. Proper identification was posted at the access road entrance, the pad entrance, the wells, and tank battery. The tank battery was surrounded by a steel dike. Other production equipment on the well pad consisted of:



The surface-

0658736805 2013-02-02 Drill / Deepen / Reopen No Violations I arrived on site at 11:40 pm on 02/01/2013 to witness the cementing of the 13 3/8 surface casing. My inspection revealed that Nomac Drilling Rig # 43 has drilled a 17 1/2 hole to a total depth of 720. Wyoming Casing crew ran 708 of 54.5 pound per foot 13 3/8 casing and installed six bowspring centralizers on joints 1,2,5,8,11,14. Schlumberger cemented casing with xxx sacks of Class A (D901) cement with 2% CaCl (S001); .125 pound per sack D130 (lost circulation material); and .2% D046 (defoamer). The cement had a density of 15.6 pounds per gallon and a yield of 1.2 cubic feet per sack. They used 5.252 gallons of mix water per sack of cement. Schlumberger pumped 132.5 barrels of cement slurry and displaced the plug with 102 barrels of water. There was good circulation throughout the job and there was approximately 17 barrels of cement was circulated to surface. They bumped the plug at approximately 08:50 am with 400 psi pressure. Could not conduct the casing test because the threads were leaking on the cement head and the 13 3/8 casing collar. The floats held and there was no flow-back. My inspection revealed no violations at this time.
119844517 2015-9-28 0:00: PL No Violations
1359793076 2012-12-21 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 conductor casing. My inspection revealed that Express Energy Services has drilled a 26 to a total depth of 120 and ran 120 of 94 pound per foot 20 casing. Express Energy Services cemented the casing with 245 sacks of Class A cement. The cement had 2% CaCl and .5 pound per sack floseal as additives for acceleration and lost circulation. The cement had a density of 15.6 pound per gallon and a yield of 1.18 cubic feet per sack. Express pumped 51.49 barrels of cement slurry and then displaced the slurry with 39.07 barrels of freshwater. There was good circulation throughout the job and approximately 18 barrels of cement was circulated to surface. My inspection revealed no violations at this time.
