0546049500 | 2013-04-30 | Administrative Inspection | No Violations | I was onsite to cement the Putnam 1H well and checked the status of this previously drilled well. The well is currently capped and is static. The cellar is full of water and the well is covered by steel grating. |
0640575759 | 2013-07-12 | Drill / Deepen / Reopen | No Violations | I was onsite to check the erosion controls of this wellsite following strong storms that made it through the region the previous week. There were no issues on the pad and all erosion controls are working properly. The wells on this site are currently shut-in as they are waiting for the completions crew to come and hydraulically fracture the wells. All the six wells have well caps on them and the cellars are watertight as they have not been pumped out since the rains came through. There are no violations at this time. |
1135753289 | 2018-11-05 | Production Wells | No Violations | The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed zero psi, and the intermediate/production annular gauge displayed 240 psi. I performed a walk-around inspection of the pad and erosional controls appeared functioning. No violations were observed at the time of inspection. |
1235884339 | 2015-01-14 | Production Wells | No Violations | I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 8, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:
Jersey barriers are installed around the production equipment. Lightening protection is installed on top of the production tanks. The well is properly identified with a clear, legibly marked well identification sign. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. The lease road is stable and drivable. No violations found. |
1363968775 | 2013-03-22 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 20 conductor casing. My inspection revealed that Precision Rathole drilled a 26 hole to 16 and then finished drilling a 24 hole to a total depth of 110. They ran 110 of 20 conductor casing (94 pound per foot / inside diameter of 19.124) with aprroximately 1 sticking up above ground level. O-TEX cemented the casing with 200 sacks of ClassA (2% CaCl, 1/2pound per sack cello-flake, and 0.4% C-41P. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.2 gallons of mix water per sack. They pumped 42 barrels of cement slurry and had 10 barrels of cement returns to surface. My inspection revealed no violations at this time |
1365524399 | 2013-04-08 | Administrative Inspection | No Violations | I was contacted by Nomac 72 letting me know that they have been moved onto location and that they are preparing to spud the well 4-9-2013. We also discussed the permit, timetable and the prespud conditions for the location. There are 4 planned wells on this location. |
1365611458 | 2013-04-10 | Drill / Deepen / Reopen | No Violations | I was onsite to check the status of Nomac 72 in drilling the surface hole. They have currently just hit their TD and are preparing to pull out of the hole, lay down the BHA and load the hole so that they can log and run surface casing. They estimate that the cementing of the surface casing will occur around 8pm tonight. |
1365692168 | 2013-04-11 | Drill / Deepen / Reopen | No Violations | I was onsite to witness the cementing of the surface casing by Schlumberger on Nomac 72. There was a delay in getting the job started because of the high lightning activity in the area. The job commenced and was pumped as planned. The pump truck had some issues keeping cement at 15.6ppg weight, but the weighed sample was 15.6ppg before the cement was pumped. A total of 22 barrels of good cement were returned to surface. Returns were taken in the cellar and a weighed sample of cement taken 30 barrels before the plugged was seated weighed in at 13.5ppg. They are going to nipple up the BOP and weight 8 hours on cement before they trip back into the hole. |
1366042436 | 2013-04-15 | Drill / Deepen / Reopen | No Violations | I was onsite to check the status of the drilling by Nomac 72 on this well and to pick up previous cement job paperwork. The rig is currently taking a survey at 5800 and will continue to drill ahead following the operation. I also was onsite to check the testing of the blow out preventer and casing pressure tests. Both the BOP and the casing passed the testing conducted. There are no violation at this time. |
1373291655 | 2013-07-03 | Drill / Deepen / Reopen | No Violations | I was onsite to update the GPS coordinates for this wells as well as check the annulus pressure of these 6 wells drilled by Nomac 72. It is unsure when the wells be hydraulically fractured at this time. Currently there was a vacuum truck on location sucking water off the pad. There was also a Stonebridge crew onsite removing the rig containment that was leftover. All the wells were checked and none showed any signs of pressures on the wells. The wells are currently static and there are no violations at this time. |
1376949692 | 2013-08-12 | Production Wells | No Violations | I was onsite to check for the possibility of runoff from the drilling pad running into a local residence pond. The property and the ponds were not clearly identified in the report and I checked some in the area. The ponds appear to have no issues and the pad is currently dry and does not show any signs of substantial runoff. A clearing has been made near the pad for what looks like a pipeline right away. An inspection will occur more thoroughly when more information is received. |
1377012802 | 2013-08-19 | Production Wells | No Violations | |
1443314071 | 2017-08-02 | Production Wells | No Violations | I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #3H well is in production and is equipped with a plunger lift unit. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.
The equipment on the pad consisted of the following: 6 producing wells 1 Low pressure vertical separator 6 sand separators 6 gas processing units 1 combustor unit The lease road was stable. No violations were noted at the time of inspection. |
1511119722 | 2013-04-23 | Drill / Deepen / Reopen | No Violations | I was onsite to check the status of Nomac 72 drilling their 8.5 lateral. The rig has TD the well at 15120 and is currently running their clean up cycle. They will trip out of the hole, rig up casing crew, run 5.5 casing and cement the string sometime tomorrow. |
1715428493 | 2013-03-21 | Drill / Deepen / Reopen | No Violations | I was on site to check the status of Precision Rathole. My inspection revealed that Precision Rathole is drilling the Putnam # 3 and # 10 conductor holes. They are going to drill to approximately 110 on both holes. They advised that they would be ready to run the conductor casing in the morning and should be ready to cement around 10:00am tomorrow morning. My inspection of the site revealed no violations at this time. |
2048327932 | 2015-07-01 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:
