0071599928 | 2014-04-14 | Production Wells | No Violations | Follow up to Complaint #4643. Inspection in front of the house today revealed no observable noise or vibration from the well pad. I will continue to monitor the situation. |
0126358745 | 2014-04-16 | Production Wells | No Violations | Follow up to Complaint #4643. Inspection in front of the house today revealed no observable noise or vibration from the well pad. I could here the compressors at the bottom of the lease road. The combustor was functioning and causing minimal vibration on the pad. I will continue to monitor the situation. |
0275365117 | 2013-08-12 | Production Wells | No Violations | I was onsite to check for the possibility of runoff from the drilling pad running into a local residence pond. The property and the ponds were not clearly identified in the report and I checked some in the area. The ponds appear to have no issues and the pad is currently dry and does not show any signs of substantial runoff. A clearing has been made near the pad for what looks like a pipeline right away. An inspection will occur more thoroughly when more information is received. |

0420785604 | 2013-03-22 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 20 conductor casing. My inspection revealed that Precision Rathole drilled a 26 hole to 16 and then finished drilling a 24 hole to a total depth of 110. They ran 110 of 20 conductor casing (94 pound per foot / inside diameter of 19.124) with aprroximately 2 sticking up above ground level. O-TEX cemented the casing with 200 sacks of ClassA (2% CaCl, 1/2pound per sack cello-flake, and 0.4% C-41P. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.2 gallons of mix water per sack. They pumped 42 barrels of cement slurry and had 10 barrels of cement returns to surface. My inspection revealed no violations at this time. |
0432911207 | 2013-06-04 | Casing String Witnessed | No Violations | I was onsite to witness 13554 feet of 5.5 production casing be cemented by Schlumberger. The job was cemented as planned and the cement was up to proper weight but there were several delays in the job. Twice during pumping the tail cement the Schlumberger cementing iron near the cement head leaked. This required a shutdown and a Nomac hand being raised to tighten down the connection. The delays did not affect the cement. In total 20 barrels of cement were returned to surface. Nomac 72 will be walking the rig to the 8H well as soon as the slips are set. |
0757117336 | 2018-11-01 | Production Wells | No Violations | The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed zero psi, and the intermediate/production annular gauge displayed 400 psi. I performed a walk-around inspection of the pad and erosional controls appeared functioning. No violations were observed at the time of inspection. |
0889962382 | 2013-07-12 | Drill / Deepen / Reopen | No Violations | I was onsite to check the erosion controls of this wellsite following strong storms that made it through the region the previous week. There were no issues on the pad and all erosion controls are working properly. The wells on this site are currently shut-in as they are waiting for the completions crew to come and hydraulically fracture the wells. All the six wells have well caps on them and the cellars are watertight as they have not been pumped out since the rains came through. There are no violations at this time. |
0955899318 | 2013-07-03 | Drill / Deepen / Reopen | No Violations | I was onsite to update the GPS coordinates for this wells as well as check the annulus pressure of these 6 wells drilled by Nomac 72. It is unsure when the wells be hydraulically fractured at this time. Currently there was a vacuum truck on location sucking water off the pad. There was also a Stonebridge crew onsite removing the rig containment that was leftover. All the wells were checked and none showed any signs of pressures on the wells. The wells are currently static and there are no violations at this time. |
0968497556 | 2017-08-02 | Production Wells | No Violations | I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #10H well is in production and is equipped with a plunger lift unit. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.
The equipment on the pad consisted of the following: 6 producing wells 1 Low pressure vertical separator 6 sand separators 6 gas processing units 1 combustor unit The lease road was stable. No violations were noted at the time of inspection. |

1323737126 | 2014-08-07 | Production Wells | No Violations | Follow up to Complaint #4643 with supervisor Tom Hill. Inspection in front of the house today revealed a slightly observable, low frequency noise from the well pad. Inspection of the well pad found the combustor creating a minor, vibrating energy through the louvers. Tom talked to the complainant and she said the situation was much improved since April. Tom is going to speak with Chesapeake about altering the set up and design of the combustor in order to mitigate the energy pulse emanating from through the louvers. I will continue to monitor the situation. |
1354807949 | 2014-04-08 | Production Wells | No Violations | I was on location with Tom Hill to follow up on Complaint # 4643 concerning vibrations at a home 0.3 miles northwest of this pad and being caused by production operations. This well and 5 other horizontal wells are being placed into production on this pad including PN#s 22295, 22296, 22297, 22299, & 22300. We went by the complainants home and observed a definite, low frequency sound/energy pulse emanating from the Putnam pad, much like what would be generated by a sub-woofer. Tom and I met with the Chesapeake production supervisor to inspect operations. Each well is producing through a dedicated sand separator and gas production unit. Liquids pass to a heated oil stabilizer to separate the heavier gases out of the produced oil. The heavier fractions are produced to 2 large LNG vessels and the oil is produced to a tank battery containing 8-400 barrel tanks. There are 2 compressors running on location. One is keeping the LNG vessels pressurized in order to keep the heavier fractions in the liquid state. The other is pressurizing the sales line to move the produced natural gas to market. We found the source of the vibrations emanating from the louvers on the vapor combustor that incinerates the residual, heavy gases not removed in the stabilizer. We were told that they were having trouble maintaining the heat in the stabilizer and that a more than expected heavier fraction was making it to the tanks and combustor. Chesapeake is working to correct the issue and reduce the low-frequency energy emanating from the combustor. I will check back next week on progress. |

1440432071 | 2015-07-01 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:

Other production equipment on the well pad consisted of:
Jersey barriers are installed around the production equipment. Lightening protection is installed on top of the production tanks. The well is properly identified with a clear, legibly marked well identification sign. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. The lease road is stable and drivable. No violations found. |
