0110452739 2013-05-11 Administrative Inspection No Violations I was on the phone with Nomac 72 regarding the Putnam 5H well. The rig has skidded and is going to spud the well today. They are going to set surface casing to around 610. They are going to call me when they start running casing so that I can witness the cementing of the surface casing.
0257849736 2013-08-12 Production Wells No Violations I was onsite to check for the possibility of runoff from the drilling pad running into a local residence pond. The property and the ponds were not clearly identified in the report and I checked some in the area. The ponds appear to have no issues and the pad is currently dry and does not show any signs of substantial runoff. A clearing has been made near the pad for what looks like a pipeline right away. An inspection will occur more thoroughly when more information is received.
0460888243 2015-07-01 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0492857826 2017-08-02 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #5H well is in production and is equipped with a plunger lift unit. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consisted of the following: 6 producing wells

1 Low pressure vertical separator 6 sand separators 6 gas processing units 1 combustor unit The lease road was stable. No violations were noted at the time of inspection.

0526549763 2013-07-03 Drill / Deepen / Reopen No Violations I was onsite to update the GPS coordinates for this wells as well as check the annulus pressure of these 6 wells drilled by Nomac 72. It is unsure when the wells be hydraulically fractured at this time. Currently there was a vacuum truck on location sucking water off the pad. There was also a Stonebridge crew onsite removing the rig containment that was leftover. All the wells were checked and none showed any signs of pressures on the wells. The wells are currently static and there are no violations at this time.

1364488721 2013-03-28 Drill / Deepen / Reopen No Violations I was on site to witness the cementing on the 20 conductor on the 5H and 1H wells. My inspection revealed that they were runnng the 20 conductor casing into the Putnam 1H well. The advised that as soon as they finished running the conductor csing into the 1H well, they would be ready to cement the conductor casing on this well (# 5H). My inspection revealed that Precision Rathole has drilled a 26 hole to 16 and drilled a 24 hole to a total depth of 110. They ran 110 of 94 pound per foot 20 conductor casing (inside diameter 19.124) and O-TEX cemented the casing with 200 sacks of ClassA 2% CaCl, 1/2 pound per sack cello-flake, and 0.4% C-41P. The cement had a density of 15.6 pounds per gallon, a yield of 1.18, and used 5.24 gallons per sack mix water. They pumped 42.03 barrels of cement slurry and displaced the slurry with 35 barrels of freshwater. There was good circulation throughout the job and approximately 9 barrels of cement was circulated to surface. My inspection revealed no violations at this time.
1368631298 2013-05-14 Casing String Witnessed No Violations I was onsite to witness the cementing of the 9 5/8 intermediate casing set to 1958. I arrived on location as Schlumberger began to mix the gel spacer. Cement was scaled at 15.7ppg before it was pumped down hole and 40 barrels of 15.3ppg cement was returned to surface. The rig will continue to air drill its vertical section after 8 hours of waiting on cement.
1397374968 2015-01-14 Production Wells No Violations I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 8, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. 1642157923 2018-11-01 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed zero psi, and the intermediate/production annular gauge displayed 390 psi. I performed a walk-around inspection of the pad and erosional controls appeared functioning. No violations were observed at the time of inspection.
1908108664 2013-05-12 Casing String Witnessed No Violations I was onsite to witness the cementing of the 13 3/8 surface casing that was set at 613. There were some issues drilling the hole as they had some bad drill string that needed to be addressed. Also there was a delay in the cement job when the Schlumberger bulk cement truck broke down on the drive to the location. Another issue occurred when there was no vacuum truck on location like there was supposed to be. The job began and returns were taking into the cellar. The pump on location could not handle the cement and the cellar filled up and overflowed slightly. A pump truck made it to location and was able to pump down the cement. Cement was returned to surface but an exact amount cannot be calculated due the cellar over flowing. Also a sample could not be taken to be weighed due to the overflow. The cement job did go as well, the floats held and cement was returned to surface as required.
2125086709 2013-07-12 Drill / Deepen / Reopen No Violations I was onsite to check the erosion controls of this wellsite following strong storms that made it through the region the previous week. There were no issues on the pad and all erosion controls are working properly. The wells on this site are currently shut-in as they are waiting for the completions crew to come and hydraulically fracture the wells. All the six wells have well caps on them and the cellars are watertight as they have not been pumped out since the rains came through. There are no violations at this time.
