0048880492 2013-06-17 Drill / Deepen / Reopen No Violations I was onsite to check the status of drilling the 12.5 intermediate hole. Nomac 72 has currently TD the well and pulling the bottom hole assembly out of the hole at this time. They will run a CBL over the 13 3/8 casing and will begin to run casing. The cement job will occur later tonight for which I will be witnessing.
0138036856 2013-07-03 Drill / Deepen / Reopen No Violations I was onsite to update the GPS coordinates for this wells as well as check the annulus pressure of these 6 wells drilled by Nomac 72. It is unsure when the wells be hydraulically fractured at this time. Currently there was a vacuum truck on location sucking water off the pad. There was also a Stonebridge crew onsite removing the rig containment that was leftover. All the wells were checked and none showed any signs of pressures on the wells. The wells are currently static and there are no violations at this time.
0262971127 2013-06-17 Casing String Witnessed No Violations I was onsite to witness the cementing of the 9 5/8 intermediate casing set to 1967 feet. The job was pumped as planned and a total of 8 barrels of cement weighing 15.2ppg were returned to surface. The cement was weighed at 15.6ppg before it was pumped down hole. The floats held and .5 barrels returned to the pump truck. There are no violations at this time.
0455321974 2015-07-01 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

They have currently their desired depth of 14105 and are beginning to trip out of the hole with the drill pipe. They expect to be cementing the 5.5 production casing early in the mourning.

0765680656 2013-07-12 Drill / Deepen / Reopen No Violations I was onsite to check the erosion controls of this wellsite following strong storms that made it through the region the previous week. There were no issues on the pad and all erosion controls are working properly. The wells on this site are currently shut-in as they are waiting for the completions crew to come and hydraulically fracture the wells. All the six wells have well caps on them and the cellars are watertight as they have not been pumped out since the rains came through. There are no violations at this time.
0926908415 2013-06-25 Casing String Witnessed No Violations I was onsite to witness the cementing of the 5.5 production casing set to a depth of 14105 feet. The job was pumped as planned and the plug was currently seated in the shoe. In total 60 barrels of mudpush came back to surface. The rig will be released and move to the Smith Nursery pad at 6pm today.
1083832974 2013-06-16 Casing String Witnessed No Violations I was onsite to witness the cementing of the 13 3/8 surface casing set to 610 feet for this well. The cement job was pumped as planned and in total 25 barrels of good cement returned to surface. The cement weighed in at 15.6ppg before it was pumped down hole. The intermediate casing should occur on the next 36 hours if things go as planned.
1321962729 2013-08-12 Production Wells No Violations I was onsite to check for the possibility of runoff from the drilling pad running into a local residence pond. The property and the ponds were not clearly identified in the report and I checked some in the area. The ponds appear to have no issues and the pad is currently dry and does not show any signs of substantial runoff. A clearing has been made near the pad for what looks like a pipeline right away. An inspection will occur more thoroughly when more information is received.
1422880957 2015-01-14 Production Wells No Violations I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 8, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:

1551031199 2018-11-01 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed zero psi, and the intermediate/production annular gauge displayed 210 psi. I performed a walk-around inspection of the pad and erosional controls appeared functioning. No violations were observed at the time of inspection.
1773037849 2013-06-20 Drill / Deepen / Reopen No Violations I was onsite to check the status of Nomac 72 drilling. Currently the rig is picking up its curve assembly to drill its 8.5 curve and lateral hole. It was also of concern for Nomac 72 whether or not Smith Nursery, the next drilling location for Nomac 72, would be completed in time. The rig is several days off from finished the well and being ready to move to the next location. The company man gave me the coordinates for the next location and I will conduct an inspection on it to check the status of the construction. There are no violations at this time.
