0327864313 2015-11-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. The well was in production at the time of this inspection, and the status needs changed in RBDMS from DR to PR. Preliminary restoration passed.

0499130214 2014-11-05 Drill / Deepen / Reopen No Violations Tophole Drilling drilled a 26 hole to a depth of 120 and ran 120 of 20 94 #/ft. welded conductor casing. Express Energy Services pumped 10 barrels of fresh water ahead followed by 230 sacks of class A cement with 2% CaCl and .05 ppg of floseal. Displaced with 39 barrels of fresh water. 11 barrels of cement returned to surface.

1288026492 2018-09-17 Production Wells No Violations No equipment changes have been made to the pad since my last inspection. The surface-intermediate annular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 52 psi. I performed a walk-around inspection of the pad, and no violations were observed at this time.
1354175082 2018-11-08 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 213 psi. I performed a walk-around inspection of the production pad and no violations were observed at the time of inspection.
1423511589 2015-02-07 Casing String Witnessed No Violations I went to the site to witness the cementing of the surface casing. The drilling contractor for Chesapeake is Nomac Rig 77. Nomac drilled a 17.5 diameter hole to 945. There were no issues while drilling the surface hole. The 13.375 OD surface casing was set at 930 with a 27 landing joint. A total of 8 centralizers were installed on the surface casing string. O-Tex was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement was mudscaled at 15.6 lbs per gallon (ppg). O-Tex pumped 179.5 bbls of cement slurry at 15.6 ppg. Cement was mudscaled two other times during the job and both times the cement mudscaled at 15.6 ppg. Cement returns to surface were observed. An estimated 37 bbls of cement returned to surface. The cement returns were mudscaled at 15.5 ppg. The plug was displaced with 137 bbls of water. The plug bumped the float collar and O-Tex pressured up on the casing and held the pressure for 5 minutes. The float held and approximately 1 bbls of water bled back when the pressure was released. The permit was in the company mans trailer. There were multiple fire extinguishers present during the cement job. The location was in stable condition. No violations found.
1423573922 2015-02-08 Casing String Witnessed No Violations I went to the site to witness the cementing of the intermediate casing. The drilling contractor for Chesapeake is Nomac Rig 77. Nomac drilled a 12.25 diameter hole to 2280. There were no issues while drilling the intermediate hole. The 9.625 OD surface casing was set at 2665 with a 27 landing joint. A total of 17 centralizers were installed on the intermediate casing string. O-Tex was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement was mudscaled at 15.8 lbs per gallon (ppg) and O-Tex pumped 172 bbls of cement slurry at 15.8 ppg. Cement was mudscaled two other times during the job and both times the cement mudscaled at 15.8 ppg. Cement returns to surface were observed. An estimated 36 bbls of cement returned to surface. The cement returns were mudscaled at 15.2 ppg. The plug was displaced with 165 bbls of water. The plug bumped the float collar and O-Tex pressured up on the casing and held the pressure for 5 minutes. The float held and approximately 0.5 bbls of water bled back when the pressure was released. The permit was in the company mans trailer. There were multiple fire extinguishers present during the cement job. The location was in stable condition. No violations found.
1424357138 2015-02-11 Drill / Deepen / Reopen No Violations I performed a status check on the above location. Nomac Rig 77 drilled a 8.750 hole to a depth of 7083. At present they are tripping out of the hole with there 8.750 bit so they can pick up there bottom hole assembly and start drilling there curve. All permits and conditions are posted. No noted violations at this time.
1424366004 2015-02-17 Drill / Deepen / Reopen No Violations Nomac Rig 77 drilled a 8.5 hole to a depth of 17830 at this time they are running 5.5 20 lb./ft. P-110 production casing. At this time they have 8200 in the hole. I talked to Mackey Lambert about the permit conditions on the next location they are gone to move to is in Harrison County. They will notify me when they get close to start cementing the 5.5 casing.
1424371213 2015-02-18 Drill / Deepen / Reopen No Violations Nomac Rig 77 drilled a 8.5 hole to a vertical and horizontal depth of 17830 and ran 17830 of 5.5 20 lb./ft. P-110 production casing. O-Tex Cementing was the cementing contractor. O-Tex pumped 50 barrels of barite spacer followed by 920 sacks (277 barrels ) of 13 lb./gal of lead cement, followed by 2470 sacks (559 barrels) of 15.3 lb./gal. of tail cement. They displaced with 394 barrels of water. Plug landed and pressure was increased to 3170 psi. plug held. There was 26 barrels of spacer returned to surface. Job completed.
2121481910 2018-04-23 Production Wells No Violations I conducted a status check of the pad. The pad is equipped with eight producing wells, and partially drilled wells with conductor casing (# 22624 & 22623). The surf/int pressure gauge displayed a reading 0 psi, and the int/prod displayed 0 psi. The tank battery consisted of six -400 bbl production tanks. Other equipment on the pad consisted of the following.




The access road was stable. No violations were observed at the time of inspection.
