0425050358 2015-12-10 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:


Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0497463375 2016-02-22 Preliminary Restoration No Violations I was on location for a status check. My inspection found the access road gate is locked and the access road is in good repair. Identification signs are posted at the access road entrance, well and storage tanks. The pad is fenced and all man doors were locked. The #6H is in production. The well is equipped with a plunger lift assembly. There is a steel dike around the storage tanks.

The equipment on the pad consists of the following: 6 Storage tanks 2 Sand separators 2 Gas processing units (GPU) 1 Vertical separator 1 Vapor recovery unit (VRU) 1 Thermal electric generator (TEG) 2 Production wells on the pad Site fenced and man doors locked

1364992591 2013-04-03 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 conductor casing. My inspection revealed that Precision Rathole has drilled a 26 hole to a depth of 32 and finished drilling a 24 hole to 115. Precision ran 115 of 20 conductor casing with windows cut in the casing 5 feet off bottom. O-TEX cemented the casing with 250 sacks of Class A cement (2% CaCl, 0.5 pound per sack cello flake, and 0.4% C-41P). The cement had a density of 15.6 pounds per gallon, yield of 1.18 cubic feet per sack, and they used 5.2 gallons of mix water per sack. O-TEX pumped 52.53 barrels of cement slurry and displaced the cement with 37.3 barrels of freshwater. There was good circulation throughout the job and approximately 10 barrels of cement was circulated to surface. My inspection revealed no violations at this time.
1528982097 2015-04-28 Production Wells No Violations I meet with the Federal EPA at this location. They performed an Air Quality Inspection of the well and tank battery. I performed a status check of the tank battery and well head. ID signs are posted. No violations were noted at this time.
