0839104130 | 2015-01-20 | Production Wells | No Violations | I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 8, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:

1235002476 | 2017-10-16 | Production Wells | No Violations | I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. Three producing wells were located on the pad #(22429, 22430, 22433). The well is equipped with a plunger lift unit. There were also three wells on site that were drilled to conductor depth and were capped #(22431, 22432, 22434).
The equipment on the pad consists of the following: 6 - 400 bbl production tanks, three labelled as containing produced water, and three labelled as containing condensate. The tanks were surrounded by a steel dike. 1 Low pressure vertical separator 6 Sand separators 6 Gas processing units 1 Heat Treater 1 Combustor unit. The access road was stable and drivable. No violations were noted at the time of inspection. |
1379082250 | 2013-09-12 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 13 3/8 inch surface casing. I arrived on site at approximately 8:35pm (9/11/13) Wyoming Casing crew was running the 13 3/8 inch surface casing. My inspection revealed that Nomac Rig # 72 has drilled a 17.5 inch hole to a total depth of 530 feet and ran 505 feet of 13 3/8 inch casing. Seven bowspring centralizers were installed on the casing string (on joints 1,2,5,7,9,11, and on the landing collar. Schlumberger cemented the casing with 460 sacks of ClassA (D-901) 0.4% D-400, 0.4% D202, 0.2% D046, 1.0% D154, 1.5% DS001, and 1/8 pound per sack D130. The cement had a density of 15.6 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.24 gallons of mix water per sack of cement. Schlumberger pumped 99 barrels of cement slurry, pumped 73.8 barrels of displacement, and there was 22 barrels of cement returns to surface. They bumped the plug at 12:01am (09/12/13) with 237 PSI and then pressured up to 826 PSI to conduct the casing test for 5 minutes. Casing test was approved at 12:06am and the pressure was released and the floats held. No violations at this time. |
1723475426 | 2013-08-26 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 20 conductor casing. My inspection revealed that Precision Rathole drilled a 26 inch hole to a depth of 18 feet and ran a 26 inch sleeve to a depth of 18 feet. Precision then drilled a 24 inch hole to a depth of 120 feet and ran 120 feet of 20 inch conductor casing. Allied cemented the casing with 230 sacks of Class A 2% CaCl and 1/2 pound per sack cello flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and 5.2 gallons of freshwater per sack of cement. Allied pumped 48 barrels of cement slurry, 39 barrels of freshwater displacement, and there was 12 barrels of cement returns to surface. My inspection revealed no violations at this time. |
2098202568 | 2015-07-01 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:
