| 0454141559 | 2015-12-23 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:
Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed. |
| 0956979032 | 2014-02-20 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 9 5/8 inch intermediate casing. My inspection revealed that Patterson Rig # 261 drilled a 12 1/4 inch hole to a total depth of 1816 feet and ran 1806 feet of 9 5/8 inch intermediate casing (40 pounds per foot / inside diameter is 8.835 inches). They installed 14 bowspring centralizers on joints 1,3,6,9,12,15,18,21,24,27,30,33,36, and 39. O-TEX cemented the casing with 700 sacks of Class H 3% Gypsum, 2% CaCl, 0.4% C-41P, 0.8% MC500, and 0.5% C-45. The cement had a density of 15.8 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.2 gallons of mix water per sack. O-TEX pumped 150.85 barrels of cement slurry, 130 barrels of displacement, and there was 27 barrels of cement returns to surface. They bumped the plug 12:00pm with 990 PSI and conducted the casing test with 1508 PSI. Casing test approved and the floats held when the pressure was released. My inspection of the well site revealed no violations at this time. NOTE: O-TEX pump truck broke down while pressure testing the lines so O-TEX had to send another pump truck to the site. |
