0245728129 2015-01-13 Production Wells No Violations I was on location to conduct a status check of this well. The well is open flow with a wellhead tree assembly. Jersey barriers were installed around the wellhead. The tank battery consisted of 6, 400 BBL production tanks and was diked. A windsock was installed on the top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. Lightening protection is installed on top of the production tanks. The well is properly identified with a clear, legibly marked well identification sign. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. The lease road is stable and drivable. No violations found.

0603542000 2015-07-01 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1270924512 2017-10-30 Production Wells No Violations I conducted a status check of this well. Identification was posted at the access entrance, the wells, and tank battery. The well is in production and was equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: -400 bbl production tanks, three labelled as containing condensate, and three labelled as containing produced water -pressure vertical separator



The access road was in good repair. I did not locate a surface-intermediate annular gauge. I contacted Chesapeake to have a gauge installed.

1372441406 2013-10-06 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Express Energy Services has drilled a 26 inch hole to a total depth of 120 feet and ran 20 inch conductor casing to a depth of 120 feet. Allied cemented the casing with 236 sacks of ClassA cement (2% CaCl and 1/2 pound per sack floseal). The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.2 gallons of mix water per sack. Allied pumped 52 barrels of cement slurry, pumped 39 barrels of freshwater displacement, and there was 11 barrels of good cement returns to surface. My inspection revealed no violations at this time.
