0067772000 2017-06-15 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #7H well is in production and is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. A sound barrier wall was installed on the north side of the pad.

The equipment on the pad consists of the following: 7 producing wells

7 sand separators 7 gas processing units 1 vapor recovery unit 1 heat treater

No violations were noted at the time of inspection.

0562654488 2018-02-22 Production Wells No Violations I conducted a status check of the pad with DOGRM District Supervisor Stephen Ochs. The surface-intermediate and intermediate-production annular gauges both displayed a reading of zero psi. No violations were observed at the time of inspection.
0761724646 2018-06-27 Production Wells No Violations I conducted a status check of the well and production pad. Seven producing wells are located on the pad (permit numbers (2528, 2530, 2498, 2554, 2486, 2553, 2529). This well is equipped with plunger-lift configuration. The surface-intermediate annulus was equipped with a pressure-relief valve and a pressure gauge which displayed a reading of zero psi. Identification was posted at the well and lease entrance. The production tanks and production equipment were surrounded by a lined steel dike. Less than an inch of standing water was present in areas inside the tank battery. I tested the standing water for chlorides using a low-range QuanTab which concluded in a reading of less than 33 ppm. The access road was stable and drivable. Erosional controls were in place and appeared functioning.

Equipment on the pad consisted of the following: Fifteen 400 bbl production tanks; eight labelled as containing condensate and seven as containing produced water Seven gas processing units with vertical separators Two combustor units Three vapor recovery units One oil stabilization unit and tower

1132056668 2016-01-27 Work Over Reconditioning No Violations I was on the Grunder Pad to review the status of the wells shut-in due the drilling of the Grunder # 2H. A kill plug placed at approximately 6500 has leaked and the pressure has equalized across the plug. Surface pressure were 1600 psi. Removal is not anticipated to be a problem because of the pressure equalization across the plug. But methane hydrates might be precipitated on top of this plug might hinder removal. Due to working conditions an inspection of the annular pressures could not be performed. No violations identified during this inspection.
1382369851 2013-10-21 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Sidewinder Drilling Rig # 63 has drilled a 17.5 inch hole to a total depth of 1016 feet and ran 1015 feet of 13 3/8 inch (54.5 pound per foot/ inside diameter of 12.615 inches) casing into the wellbore. They installed 8 centralizers on the casing string (on joints 1,2,6,10,14,18,22,25). Universal cemented the casing with 802 sacks of Type 1 3% CaCl, 0.2% foam check, and 1/4 pound per sack flake. They pumped 171 barrels of cement slurry, pumped 149 barrels of displacement, and there were 31 barrels of cement returns to surface. Plug bumped at 4:20pm and they pressured up on the casing. Universal released the pressure and the floats held. My inspection revealed no violations at this time.
1382595860 2013-10-24 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 inch intermediate casing. My inspection revealed that Sidewinder Rig # 63 has drilled a 12 1/4 inch hole to a total depth of 2045 feet and ran 2045 feet of 9 5/8 inch casing (40 pounds per foot / inside diameter 8.835 inches). They installed 14 centralizers on the casing string ( on joints 1,2,6,10,14,18,22,26,30,34,38,42,46, and 47). Universal cemented the casing with 635 sacks of Gas Block 2% CaCl and 0.15% CR3. The cement had a density of 15.7 pounds per gallon, a yield of 1.29 cubic feet per sack, and they used 5.54 gallons of mix water per sack. Universal pumped 146 barrels of cement slurry, pumped 154 barrels of displacement, and there were 34 barrels of cement returns to surface. They bumped the plug with 1000 psi and increased the pressure to 1520 psi. to check the plug and casing integrity. Note: The pressure held and the test was approved. My inspection revealed no violations at this time.
1904742053 2019-01-31 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate and intermediate/production annular gauges displayed zero psi. The well was recently transferred to PennEnergy Resources LLC, and updated identification was posted. No violations were observed at the time of inspection.
2055128113 2017-06-29 Production Wells No Violations I was on site with DOGRM Tom Hill, Bob Worstall, and Terry Whitehair, along with R.E Gas Development personnel to drilling an additional three wells (PN# 22553, 22732, 22733) on the pad. I performed a walk-around inspection while on site. The well is equipped with a plunger-lift assembly. I was unable to locate a clear surface-intermediate pressure gauge on the well. I spoke to R.E. Gas Development to have gauges installed on the surface-intermediate annulus and labelled.
