0124722693 | 2017-10-30 | Production Wells | No Violations | I conducted a status check of this well. Identification was posted at the access entrance, the wells, and tank battery. The well is in production and was equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.
The equipment on the pad consists of the following: -400 bbl production tanks, three labelled as containing condensate, and three labelled as containing produced water -pressure vertical separator

0184019674 | 2013-11-22 | Drill / Deepen / Reopen | No Violations | I was on location with inspector Kevin Perry to inspect drilling operations of Nomac Rig #73. They are preparing to rig down and move to the Lucas drilling pad. All equipment is on contaiment. No violations. |
0595234831 | 2018-10-16 | Production Wells | No Violations | The well is equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed zero psi and the intermediate/production annular gauge displayed 350 psi. No changes have been made to the production equipment since my last inspection. Erosional controls appeared to be in place and functional. No violations were observed at the time of inspection. |
0936371723 | 2015-07-01 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock is installed on top of the production tanks. Other production equipment on the well pad consisted of:

1598451175 | 2013-10-06 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Express Energy Services has drilled a 26 inch hole to a total depth of 120 feet and ran 20 inch conductor casing to a depth of 120 feet. Allied cemented the casing with 236 sacks of ClassA cement (2% CaCl and 1/2 pound per sack floseal). The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.2 gallons of mix water per sack. Allied pumped 51 barrels of cement slurry, pumped 39 barrels of freshwater displacement, and there was 14 barrels of good cement returns to surface. My inspection revealed no violations at this time. |

Jersey barriers are installed around the production equipment. Lightening protection is installed on top of the production tanks. The well is properly identified with a clear, legibly marked well identification sign. The entire well pad is enclosed with a 6 foot high, chain link fence with 3 strand barbed wire and the man doors were locked. The lease road is stable and drivable. No violations found. |
2003630872 | 2018-10-16 | Production Wells | No Violations | The well is equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed zero psi and the intermediate/production annular gauge displayed 430 psi.
