0314651375 2013-12-14 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Express had drilled a 26 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch conductor casing (94 pounds per foot casing / inside diameter 19.124 inches). Allied cemented the conductor casing with 250 sacks of Class A cement with 2% CaCl and 1/2 pound per sack floseal. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 52.5 barrels of cement, 39 barrels of displacement, and there was 10 barrels of cement returns to surface. My inspection revealed no violations at this time.
0522194678 2015-11-23 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0569906554 2016-03-11 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was locked. The #5H well is in production and the well is equipped with a plunger lift assembly. The pad is surrounded by a fence and all man doors were locked. The production tanks are surrounded by a steel dike. There was approximately six inches of standing water contained inside of the dike. I tested the standing water for chlorides using low-range QuanTabs which resulted in a reading of 216 ppm. The equipment on the pad consists of the following:

6- 400 bbl production tanks, three labelled as containing produced water and three labelled as containing condensate. 1 Low pressure vertical separator located inside of the dike. 7 sand separators 7 gas processing units 1 vapor recovery unit 1 oil stabilization unit 3 line heaters

1393440512 2014-02-26 Drill / Deepen / Reopen No Violations Kyle Tharp and I were on location to witness the cementing of the 13 3/8 inch surface casing. Our inspection revealed that Nomac Drilling Rig # 72 has drilled a 17 1/2 inch hole to a total depth of 595 feet and ran 585 feet of 13 3/8 inch surface casing (54.5 pounds per foot / inside diameter is 12.615 inches). They installed 5 bowspring centralizers on the casing string on joints 1,4,7,10, and 13. Schlumberger cemented the casing with 530 sacks of Class A (D901) 1% D154; 0.2% D046; 0.15% B160A; 0.5 B348; 0.75% S001; .125 pound per sack D130. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.219 gallons of mix water per sack. Schlumberger pumped 113 barrels of cement slurry, 84 barrels of displacement, and there was 28 barrels of cement returns to surface. They bumped the plug with 340 PSI and then conducted the casing test with 900 PSI. The casing test was approved, pressure was released, and there was approximately 1/2 barrel of water flowed back. My inspection of the well site revealed no violations at this time.
1393563575 2014-02-28 Drill / Deepen / Reopen No Violations Kyle Tharp and I were on site to witness the cementing of the 9 5/8 inch intermediate casing. Our inspection revealed that Nomac Rig # 72 has drilled a 12 1/4 inch hole to a total depth of 1910 feet and ran 1900 feet of 9 5/8 inch intermediate casing (40 pounds per foot / inside diameter 8.835 inches). They installed 15 bowspring centralizers on the casing string on joints 1,2,5,8,11,14,17,20,23,26,29,32,35,38, and 41. Schlumberger cemented the casing with 705 sacks of ClassA Surface Set cement with 2% D178, 0.2 D046, 0.4% B348, 0.3% D202, 0.6% D013. The cement has a density of 15.8 pounds per gallon, a yield of 1.19 cubic feet per sack, and they used 4.464 gallons of mix water per sack. Schlumberger pumped 150 barrels of cement, 137 barrels of displacement, and there was 7 barrels of cement returns to surface. They bumped the plug with 880 PSI and conducted the casing test with 1410 PSI. Casing test was approved, pressure released, floats held, and there was 3/4 barrel of flowback. Our inspection revealed no violations at this time. NOTE: The drilling rig had issues with lines freezing and Schlumberger had issues with air lines on the bulk trucks freezing while trying to off-load the cement.
1535639940 2018-08-29 Production Wells No Violations I conducted a status check of the well and production facility. The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate production gauge displayed a reading of 250 psi. I performed a walk around inspection of the pad and no violations were observed at the time of inspection.

The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate/production gauge displayed 220 psi.
