0212997946 2018-06-27 Production Wells No Violations I conducted a status check of the well and production pad. Seven producing wells are located on the pad (permit numbers (2528, 2530, 2498, 2554, 2486, 2553, 2529). This well is equipped with plunger-lift configuration. The surface-intermediate annulus was equipped with a pressure-relief valve and a pressure gauge which displayed a reading of zero psi. Identification was posted at the well and lease entrance. The production tanks and production equipment were surrounded by a lined steel dike. Less than an inch of standing water was present in areas inside the tank battery. I tested the standing water for chlorides using a low-range QuanTab which concluded in a reading of less than 33 ppm. The access road was stable and drivable. Erosional controls were in place and appeared functioning.

Equipment on the pad consisted of the following: Fifteen 400 bbl production tanks; eight labelled as containing condensate and seven as containing produced water Seven gas processing units with vertical separators Two combustor units Three vapor recovery units One oil stabilization unit and tower

0832315598 2017-06-15 Production Wells No Violations rtI conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road was equipped with a gate and was locked. The #6H well is in production and is equipped with a plunger lift assembly open flow assembly. The production tanks are surrounded by a steel dike. A sound barrier wall was installed on the north sisde of the pad.

The equipment on the pad consists of the following: 7 producing wells

7 sand separators 7 gas processing units 1 vapor recovery unit 1 heat treater

No violations were noted at the time of inspection.

1390530150 2014-01-22 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Sidewinder Drilling Rig # 63 has drilled a 17 1/2 inch hole to a total depth of 1015 feet and ran 1005 feet of 13 3/8 inch surface casing (54.5 pounds per foot / inside diameter 12.615 inches). They installed 8 spiral solid body centralizers on the casing string (on joints 1,3,7,11,15,19,23, and 25). Universal cemented the surface casing with 794 sacks of Type 1 cement with 3% CaCl, 0.2% defoamer, and 1/4 pound per sack lost circulation material. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.2 gallons of mix water per sack. Universal pumped 169 barrels of cement slurry, 150 barrels of displacement, and there was 25 barrels of cement returns to surface. They bumped the plug at 02:13 pm with 495 PSI and then bumped the pressure up to 1025 for the casing test. The casing test was approved at this time and the floats held when the pressure was released. My inspection revealed no violations at this time.
1390915021 2014-01-28 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 inch intermediate casing. My inspection revealed that Sidewinder Rig # 63 has drilled a 12 1/4 inch hole to a total depth of 2305 feet and ran 2295 feet of 9 5/8 inch intermediate casing (40 pounds per foot / inside diameter 8.835 inches). They installed 20 spiral solid body centralizers on the casing string, on joints 1,2,6,10,14,18,22,26,30,34,38,42,46,50,54, 58,62,66,70, and 74. Universal cemented the casing with 705 sacks of Thixotropic cement with 5% W-60, 10% Salt, 2% CaCl, and 0.15% CR3. They pumped 162 barrels of cement slurry, 165 barrels of displacement, and there was 25 barrels of cement returns to surface. They didnt bump the plug because the wiper plug stayed in the Blackhawk cement head. They pumped 165 barrels of displacement out of the 171 barrels of calculated displacement. They finished pumping at 11:15 am, held pressure on the casing for 5 minutes and the floats held after the pressure was released. My inspection revealed no violations at this time.
1536384970 2014-10-03 Production Wells No Violations I was on location with North Region Supervisor Tom Hill to accompany specialists from the OEPA Division of Air Pollution Control so they could monitor emissions created by these producing operations. This location has 6 producing wells and 15-400 barrel tanks in containment. Rex Energy Production Foreman, Paul Arblaster, was on location and explained the producing operations. There is currently no ID signage posted at the wellheads or at the tank battery as required. Tom Hill asked Paul to get them posted immediately and install annular gauges on all of the wellheads. I will check back later to see if the IDs are posted and that the gauges have been installed.
1548958644 2019-01-31 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed zero psi, and the intermediate/production annular gauge displayed 100 psi. The well was recently transferred to PennEnergy Resources LLC, and updated identification was posted. No violations were observed at the time of inspection.
1567599786 2018-02-22 Production Wells No Violations I conducted a status check of the pad with DOGRM District Supervisor Stephen Ochs. The surface-intermediateannular gauge displayed a reading of 200 psi, and the intermediate-production annular gauges displayed a reading of zero psi. No violations were observed at the time of inspection.
1848778244 2017-06-29 Production Wells No Violations I was on site with DOGRM Tom Hill, Bob Worstall, and Terry Whitehair, along with R.E Gas Development personnel to drilling an additional three wells (PN# 22553, 22732, 22733) on the pad. I performed a walk-around inspection while on site. The well is equipped with a plunger-lift assembly. I was unable to locate a clear surface-intermediate pressure gauge on the well. I spoke to R.E. Gas Development to have a gauge installed on the surface-intermediate annulus and labelled.

1935610897 2013-12-08 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Rocky Mountain Drilling has drilled a 26 inch hole to a total depth of 110 feet and ran 110 feet of 20 inch conductor casing (78 pounds per foot / inside diameter 19.25 inches). Allied cement the casing with 200 sacks of Class A 2% CaCl and 1/4 pound per sack floseal. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 42 barrels of cement slurry, 37 barrels of displacement, and there was 8 barrels of cement returns to surface. My inspection revealed no violations at this time.
