0015972769 2017-11-01 Production Wells No Violations I conducted a status check of the well and pad. No changes have been made to the production equipment since my last inspection. The well is equipped with a plunger-lift unit and the surface-intermediate annular gauge displayed a reading of zero psi. The lease road was in good repair. No violations were observed at the time of inspection.
0415162355 2017-03-02 Preliminary Restoration No Violations

I was on location to conduct a preliminary restoration inspection. The #8H well is in production and consists of an open flow assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of twelve 400 bbl production tanks, four labeled as containing produced water, and eight labelled as containing condensate. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of: 12- 400 bbl production tanks, four labelled as containing produced water, and eight labelled as containing condensate 8 sand separators 8 gas processing units 1 low pressure vertical separator 1 oil stabilization unit 1 vapor recovery unit 1 horizontal separator, 1 compressor station Two bullet tanks Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.

0850511210 2018-11-19 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate gauge displayed zero psi and the intermediate/production gauge displayed 40 psi. I performed a walk-around inspection of the pad and no violations were observed at the time of inspection.

The surface/intermediateannular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 150 psi. No changes have been made to the production pad equipment since my last inspection. No violations were observed.
1856118071 2014-07-13 Drill / Deepen / Reopen No Violations Top Hole Drilling augured a 26 hole to a depth of 120. Allied Oil & Gas established circulation with fresh water and pumped 247 sacks of class A cement with 2% Calcium, .125 pps Floseal and displace to a depth of 110 with 39 barrels of fresh water. There was 12 barrels of cement returned to surface.
