0279926001 2015-11-17 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0487293520 2018-11-06 Production Wells No Violations The well was equipped with an open-flow assembly. The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate/production gauge displayed 210 psi. No violations were observed on the production pad at the time of inspection.
1389005698 2013-12-16 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor pipe. Note: Express was having trouble with their welder and their drilling rig. My inspection revealed that Express Energy has drilled a 26 inch hole to a total depth of 120 feet and ran 120 feet of 20 inch conductor casing (94 pounds per foot / inside diameter 19.124 inches). Allied cemented the casing with 250 sacks of Class A with 2% CaCl and 1/2 pound per sack floseal. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. They pumped 52.5 barrels of cement slurry, 39 barrels of displacement, and there was 10 barrels of cement returns to surface. My inspection revealed no violations at this time.
1389247771 2014-01-08 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 intermediate casing. My inspection revealed that Nomac Rig # 72 has drilled a 12 1/4 hole to a total depth of 1962 feet and ran 1952 feet on 9 5/8 intermediate casing (40 pounds per foot casing / inside diameter is 8.835 inches). They installed 15 bowspring centralizers on the casing string on joints 1,2,5,8,11,14,17,20,23,26,29,32,35,38, and 41. Schlumberger cemented the casing with 703 sacks of Class A Surface Set cement with 2% D154, 0.4% D400, 0.2% D046, 0.4% D202, and 0.6% D013. The cement had a density of 15.8 pounds per gallon, a yield of 1.19 cubic feet per sack, and they used 4.438 gallons of mix water per sack. They pumped 149 barrels of cement slurry, 141 barrels of displacement, and there was 12 barrels of cement returns to surface. The landed the plug at 11:48 pm with 900 PSI and then bumped the pressure up to 1400 PSI and conducted the casing test (casing test approved at this time). My inspection revealed no violations at this time.
1567707611 2014-01-06 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 inch surface casing. My inspection revealed that Nomac Rig # 72 has drilled a 17.5 inch hole to a total depth of 887 feet and ran 877 feet of 13 3/8 inch surface casing (54.5 pound per foot / inside diameter 12.615 inches). They installed 8 bowspring centralizers on the casing string(on joints 1,2,5,8,11,14,17,20). Schlumberger cemented the casing with 776 sacks of Class A 1% D154, 0.5% B348, 0.15% B160A, 0.2% D046, 0.6% S001, and .125 pound per sack D130. The cement had a density of 15.6, a yield of 1.2 cubic feet per sack, and they used 5.219 gallons of mix water per sack. They pumped 165.8 barrels of cement slurry, 132 barrels of displacement (129 barrels calculated displacement), and there was 39 barrels of cement returns to surface. They landed the plug at 06:40 am with 350 PSI. They bumped the pressure up to 825 PSI and conducted the casing test. The casing test was approved at this time. My inspection revealed no violations at this time.
